Founded in 1989, we began as advocates with a message to be heard: Persons Living with HIV and AIDS who had few resources and demanded to be treated with dignity.

Our name comes from the anonymous, well-known 19th century poem about children born on each day of the week. 'Thursday's Child as far to go.' The name spoke of hope and promise during times when there was little of either. We began as advocates and started the region's first independent HIV housing program. Today, an HIV diagnosis is a manageable chronic condition.

HIV testing is the first step all should take: knowing one's HIV status can make the difference between life and death. Those who know their status is positive can be linked to care and live a long and prosperous life; not knowing can lead to serious illness, even death. Today, Thursday's Child keeps its promise of going farther; we now offer various programs to support better health outcomes, provide for humane and essential needs, and continues its work to end the HIV/AIDS crisis.

A Case Manager contributes to AHF by providing individual client services that address issues of health inequity. This position requires the ability to identify and help solve barriers in order for our clients maintain medical care and life-saving HIV treatment. The Case Manager will meet with the client in the office of Thursday's Child and/or other locations to conduct comprehensive assessments, service plans, and linkages to medical and other support services.

A Case Manager enhances the reputation of AHF by delivering client-centered services with empathy and understanding to each individual's unique needs. This position contributes to ending the epidemic by focusing on preventative care and fostering healthy connections.

Optimizing individual health outcomes through this program will help reduce hospitalizations and HIV transmissions. A key initiative is ending the HIV epidemic, which is defined as getting to zero AIDS-related illnesses and zero HIV stigma.

To be successful as a Case Manager it is crucial to ensure that the AHF core Values and Mission stay top of mind with all that your do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The Case Manager will work as a part of a close knit team to ensure the successful outcomes of the Medical Case Management program through positive client experiences which meet individual needs and enhance public health outcomes. The Case Manager works for Thursday's Child, based in Patchogue, an affiliate of AHF.

Licenses/ Certifications; Bachelor's degree or higher; experience in HIV/AIDS services, public health, social work or case management highly preferred; bilingual English/Spanish highly preferred.
