People Incorporated is looking for a dynamic individual to join our team as Case Manager for our Adult Foster Care Program. The Care Manager assumes primary responsibility for each member on an assigned caseload including record maintenance and the provision of psychosocial and counseling assistance for the Adult Foster Care (AFC) program. This position will be responsible to coordinate services for adults with chronic health care needs. Responsible for case management of program participants while evaluating compatibility and coordinating training needs of foster families.
Conduct initial and ongoing evaluations of the AFC qualified program setting.
Obtain social history and conduct psychosocial assessment and evaluation.
Select, train, evaluate and supervise AFC caregivers in conjunction with program RN.
Participate in development, implementation and ongoing review of the AFC Plan of Care.
Conduct on-site visits with each member at the qualified setting as regulated.
Complete progress notes, maintain program member charts, and review care logs as regulated.
Participate in interdisciplinary meetings; discharge planning, transition planning and implementation.
Review and document the fire safety procedures for the qualified setting.
Asses the qualified setting annually.
Assist with obtaining information and access of health care and community services.
Access and read Agency email account and Intranet at least weekly.
Knowledge obtained through attainment of a Bachelor Degree from an accredited institution, and two years of recent experience working with elders or Adults with developmental disabilities.
Skill in communicating and working both independently and as team member.
Ability to effectively participate in design, implementation, and coordination of services.
Ability to complete and disseminate all required forms and documents.
A qualifying criminal background check and a valid drivers license are also required.
