People Incorporated is looking for a dynamic individual to join our team as Case Manager for our Adult Foster Care Program. The Care Manager assumes primary responsibility for each member on an assigned caseload including record maintenance and the provision of psychosocial and counseling assistance for the Adult Foster Care (AFC) program. This position will be responsible to coordinate services for adults with chronic health care needs. Responsible for case management of program participants while evaluating compatibility and coordinating training needs of foster families.


  • Conduct initial and ongoing evaluations of the AFC qualified program setting.

  • Obtain social history and conduct psychosocial assessment and evaluation.

  • Select, train, evaluate and supervise AFC caregivers in conjunction with program RN.

  • Participate in development, implementation and ongoing review of the AFC Plan of Care.

  • Conduct on-site visits with each member at the qualified setting as regulated.

  • Complete progress notes, maintain program member charts, and review care logs as regulated.

  • Participate in interdisciplinary meetings; discharge planning, transition planning and implementation.

  • Review and document the fire safety procedures for the qualified setting.

  • Asses the qualified setting annually.

  • Assist with obtaining information and access of health care and community services.

  • Access and read Agency email account and Intranet at least weekly.


  • Knowledge obtained through attainment of a Bachelor Degree from an accredited institution, and two years of recent experience working with elders or Adults with developmental disabilities.

  • Skill in communicating and working both independently and as team member.

  • Ability to effectively participate in design, implementation, and coordination of services.

  • Ability to complete and disseminate all required forms and documents.

  • A qualifying criminal background check and a valid drivers license are also required.

Recent Home Care Experience required Job Summary: Provides nursing care to patients and families in their homes, which supports the total person. This is accomplished within an interdisciplinary plan of care. Participates in orienting and supervising Home Care Aides. Works collaboratively with the LPN, Home Care Aide, Social Worker, P.T., P.T.A., O.T., C.O.T.A., S.L.P. and office support staff to coordinate patient care utilizing community resources as needed. Participates in staff education. Provides consultation regarding assessment, planning and implementation of care. Coordinates activities with other community, agency and/or department staff and carries out special activities on assignment. Responsibilities: * Provides superior customer service to internal and external clients, customers and patients * Assumes accountability for own professional practice in achieving patient outcomes * Accurately, promptly and thoroughly documents patient care observations, interventions and evaluations * Accountable for managing a caseload of patients to achieve optimum patient outcomes * Identifies primary diagnosis, reasons for home care, and problems based on assessment, which includes patient demonstration of functional status; then develops a plan of care that will best meet patient outcomes * Responsible for initiating, evaluating and updating the patient plan of care to achieve desired outcomes * Provides therapeutic care and patient education; makes appropriate referrals based on identified patient needs that will maximize independence and patient/family participation * Collaborates with the physician and other members of the health care team to ensure care is safe, effective and coordinated * Attends and participates in multidisciplinary team meetings to promote quality patient care, as scheduled by clinical supervisor * Implements Telehealth to enhance patient care outcomes * Meets weekly with Clinical Supervisor to review patient caseloads, assessing the quality of care, coordination of all disciplines involved and appropriate allocation of resources * Supervises and orients Home Care Aide to plan of care according to regulatory requirements * Responsible for reviewing all admission OASIS data (if SOC/ROC not done by primary clinician) and make changes as deemed appropriate * Completes all OASIS time point assessments when possible for consistency in documentation; includes patient demonstration of functional status * Collaborates with Performance Improvement staff * Successfully completes OASIS competency visit annually * Triages patient visit needs through a combination of onsite visits and telephone triage * Plans and implements nursing intervention which includes all appropriate elements of nursing care and prescribed medical or other therapeutic regimens, including implementing disease management programs * Provides appropriate care for a range of physician-ordered treatments (waived testing, pulse oximetry, wound care, IVs, phlebotomy, Telehealth, etc.) * Administers medications as prescribed; monitors for adverse effects; disposes of all discontinued medications in compliance with regulations * Provides and coordinates health teaching required by individuals and/or families so as to maintain the optimal possible level of health * Monitors patient progress to ensure accomplishment of patient goals; initiates changes as necessary and appropriate to role * Orients new patients to the range of services offered by the agency * Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively * Serves as a patient advocate, within the limits of the law * Ensures effective communication by checking voice mails twice per day and downloading PDA twice daily * Collaborates with scheduling department to ensure patients are visited according to treatment plan, and fulfilling any productivity expectation * Assumes accountability regarding communicating changes on patient schedules to Clinical Scheduler, and works with scheduling to maintain productivity levels if scheduled patient visits drop off * Attends and participates in clinical in-services, trainings, etc. * Other duties as assigned Requirements/Qualifications: * Must possess a current MA RN license. * Must start the process of obtaining NH RN license upon acceptance of offer of employment NH RN license must be obtained no later than 3 months after date of hire. * Must possess current, valid driver's license. * Graduate of an accredited school of nursing. ASN required; BSN preferred * Must have demonstrated competence in community health or med/surgical patient care setting, providing direct patient care with patients for 1-3 years. * Must be willing to work a minimum of one weekend per month and two holidays per calendar year and as department may require.
