It includes assessing, planning, implementing, coordinating, and evaluating health and community related services to meet a patient's physical, psychosocial, vocational, and health needs in a cost effective, outcomes-oriented and timely manner. As department needs change, the services may be performed at any level within the Network's continuum of services and/or sites.

JOB DUTIES & RESPONSIBILITIES:

  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

  • Performs admission review on all inpatients. Attends daily patient rounds and shares professional knowledge, making appropriate recommendations regarding clinical and programmatic needs of patients as it relates to professional knowledge base and existing treatment criteria.

  • Participates in individual patient care meetings with multidisciplinary team members and the patient/family to evaluate progress and to identify and resolve problems that may interfere with a positive patient outcome.

  • Assesses discharge planning needs to ensure a safe, timely and efficient discharge. Identifies variances during the patient's stay in order to evaluate and improve processes that affect the efficiency and quality of patient care.

  • Arranges for community services (including short and long-term placement) prior to discharge to meet patient's needs with recognition and documentation of patient choice of service providers. Demonstrates effective use of these resources within established reimbursement guidelines.

  • Provides patient education and advocacy as appropriate.

  • Maintains knowledge and awareness of mental health laws regarding patients' rights and voluntary/involuntary commitment proceedings, coordinating such proceedings with necessary personnel.

  • Maintains appropriate, timely, and effective communication with patients, families, community agencies, physicians, nursing staff, department managers, ancillary departments, and the Manager regarding variances, aspects of patient care management.

  • Facilitates insurance authorization, pre-certification, and concurrent reviews for assigned patients.

  • Maintains clinical records of all patient contact and clinical reviews, according to hospital policy and other regulatory guidelines (i.e., State, JCAHO). Documentation is completed in a professional and timely manner and includes interventions, recommendations, and referrals.

  • Demonstrates proper chain of command for work related issues/problems.

  • Completes Departmental Orientation Checklist within 90 days of employment and completes all identified competencies for job description on an annual basis.   • Attends department staff meetings (50%) and trainings relevant to position. Participates in activities which promote personal/professional development. Serves on assigned hospital and/or unit based councils.

  • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.

  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.

  • Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external).

  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.

  • Complies with Network and departmental policies regarding attendance and dress code.

PHYSICIAL AND SENSORY REQUIREMENTS:

Sitting for up to 5 hours per day, 2 hours at a time. Standing for up to 1 hour per day, 1 hour at a time. Walking for up to 2 hours per day, 20 minutes at a time. Occasionally uses fingers to turn pages. Frequently uses hands to write, type, and hold charts. Rarely uses the hands or fingers for firm grasping or twisting/turning. Frequently uses upper extremities to lift and carry up to 10 pounds. Frequently stoops, occasionally squats, rarely reaches above shoulder level. Hearing as it relates to normal conversation in person or on telephone. Seeing as it relates to general vision, near vision, far vision, color vision, and peripheral vision.

EDUCATION:

Registered Professional Nurse with current license to practice in the state of Pennsylvania. Bachelor's Degree in Nursing required, ANCC Certification preferred, OR Licensed Social Worker, MSW, Licensed Professional Counselor (LPC) required.

TRAINING AND EXPERIENCE:

At least 2 to 5 years of clinical experience providing care to patients with mental health illness. Previous experience in discharge planning and case management models essential.

JOB DUTIES AND RESPONSIBILITIES: * Accurately obtains and records patient vital signs and I & O. Performs glucometry and phlebotomy skills with competence and collects other specimens for diagnostic tests. * Monitors the therapeutic environment by performing safety checks and monitors patient status by performing patient observations (continual observations, q 15 minutes, q 30 minutes). * Demonstrates skill as a member of the Control Team, in responding to agitation/violent behavior of individuals requiring verbal de-escalation and/or physical restraint. * Demonstrates competence in caring for the agitated/violent patient, who may require treatment with seclusion/restraints. * Assumes responsibility for maintaining a clean and orderly environment. * Facilitates patient recreational and therapeutic activities under the direction of the professional licensed staff. * Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated. * Demonstrates proper chain of command for work related issues. * Maintains clinical records of all patient contact according to hospital policy and other regulatory guidelines (i.e., Department of Health, Department of Public Welfare, and the Joint Commission). PHYSICAL AND SENSORY REQUIREMENTS: Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability to frequently use fingers/hands for fingering and handling. Must have the ability to use upper extremities for frequent pushing of 3 objects up to 10 pounds and occasional pushing of objects up to 75 lbs. Must have the ability to transport patients weighing up to 250 lbs. via wheelchair, stretcher and/or bed. Must have the ability for frequent stooping and bending. Must have the ability for frequent reaching above shoulder level. Must have the ability for touching as it relates to feeling objects by touching with the skin, particularly that of fingertips. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision. EDUCATION: High School Graduate/GED required. BA/BS in human services field, or current student in BA/BS/Nursing School preferred. TRAINING AND EXPERIENCE: Two years previous experience in mental health or medical setting preferred.
