Case Manager

One of the Bay Area's most premium hospital systems is looking for TWO RN Case Managers to join their team!

Job Description:

Case Management & Care Coordination: Assess, plan, implement, and evaluate individualized patient care plans, ensuring continuity of care across the hospital and post-discharge settings.

Discharge Planning: Develop and execute comprehensive discharge plans by collaborating with interdisciplinary teams, patients, and families to ensure a smooth transition from hospital to home or other care settings.

Patient Advocacy & Education:Provide education and support to patients and families regarding post-discharge care needs, including medications, follow-up appointments, and home health services.

Resource Management & Documentation: Identify and coordinate appropriate resources and services to meet patient needs while ensuring adherence to clinical guidelines and accurate documentation for insurance, compliance, and quality measures.

Requirements:

• CA RN License
• Two years of relevant experience
• Discharge planning experience a plus
• American Heart Associate- BLS or willing to get upon offer

Schedule:

Monday- Friday, 8am- 430pm, every other weekend, 40 hours guaranteed

Contract Term:

13 week contract to start, high probability of extensions
