Case Manager I (2) Two Positions

Area Agency on Aging (AAoA)

Department of Human Services

Hourly Wage: $23.28-$26.20/Regular/Full-Time

Assist adults with disabilities, older persons, and family caregivers to assess their needs, screen for and assist persons to access and arrange needed in-home and community services or authorize and maintain in-home and community based services to: (1) maintain their independence in the community; (2) be diverted from nursing home or other institutional settings; (3) make a timely return home following a short hospital or residential stay; and (4) remain at home with support despite functional impairments. Develops and administers a service plan which will result in maintaining the client at the highest level of independent living possible while still addressing the issues which arise in acute situations.

Examples of Work Performed:

Conduct electronic version of CARE comprehensive functional assessments and reassessments.

Develops and implements a plan of care listing services recommended and describing the division of tasks between the Case Manager, support network, and the client; ensures client is referred successfully to all resources listed in the plan and follow-up to see that resources are obtained in a timely manner, appropriate to need.

Review the service summary to document significant changes in the clients condition or situation.

Determine whether resources are appropriately meeting the needs of the client and institute case coordination with community contacts and resources to ensure coordinated service delivery.

Authorize services available in the clients Care Plan and authorize payment for services utilizing the ProviderOne payment system.

Access and utilize the following DSHS data systems: Automated Client Eligibility System (ACES), the DSHS BarCode system, the ProviderOne payment system, the Independent Provider One (IPOne) system, the Washington Connection benefit portal, and the CLC/Get Care System.

Maintain ongoing contact with client to enable a prompt response to changes in living environment.

Provide termination planning when the clients informal support systems are operating adequately to meet client need or the client no longer meets case management target population criteria.

Attend Case Management staff meetings, case reviews, and inter-agency staffing, etc.

Maintain accurate, up-to-date and complete records of each client. Includes but not limited to; annual reassessments, interim assessments, significant change assessments, Service Episode Record (SER) notes, collateral contacts, financial documents, and consent forms.

Perform in the capacity as Fair Hearing coordinator for other Case Managers in the department.

Provide other case management support functions (e.g., client advocacy, assistance, consultation, networking, family support, and crisis intervention).

Performs information and assistance activities. Conducts follow-up with clients and/or family as required.

Access the CLC/GetCare System in order to manage, record, and report service provision and utilization, demographic, resource directory, and consumer website information. This may include accessing the Washington Connection portal to assist individuals in completing an online application for federal, state, and local benefits, service, and assistance.

Participate in community liaison activities which include providing information services regarding the elderly to service groups, senior centers, and gatekeepers.

Other duties as assigned.

Knowledge, Skills and Abilities:

Knowledge of the principles and practice of social work.

Knowledge of the aging process, Washington State Aging Network service delivery system, and other service delivery systems.

Participate in training designed to upgrade skills and expand fields of expertise.

Ability to understand and apply federal, state, and tribal policies & procedures and regulations applicable

to the assignment.

Ability to adhere to confidentiality requirements of client records including federal, state, and Tribal

confidentiality policies.

Ability to utilize personal and laptop computers, telephone, copy and fax machines, typewriter and adding machine. Proficient with Microsoft Office Suite.

Ability to conduct assessments remotely, including the home of the client.

Minimum Requirements:

Bachelor's degree in Social Work, Social Science, Counseling, or related field; OR any equivalent combination of experience, training, and/or education approved by Human Resources.

One year of direct service case management or social service program experience required; two years preferred.

Applicant must complete and sign a DSHS 09-653 Background Authorization form.

Appointment to position is subject to the results of the background inquiry.

Must pass a pre-employment background check.

Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.

Required to pass a pre-employment drug test.

Preferred Requirements:

Bilingual English/Spanish preferred, not required.

Two years of direct service case management or social service program experience preferred.

Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
