Overview:

THIS WORK MATTERS!

The Department of County Human Services seeks Case Manager 2 to work with those with Intellectual and Developmental Disabilities. This position will not make you a billionaire but will give you a billion rewards-the State of Seniors and People with Disabilities contracts with Multnomah County to provide case management services. As a case manager, you will access, coordinate and assure the delivery of services and supports required by individuals with developmental disabilities.

We have teams that work with Children, High School Transition, Foster Care, Adults, and Seniors who need your help accessing and receiving services within their community. We strongly encourage you to apply if you can speak a second language.

As the Case Manager 2 ,other duties will include:

Primary responsibilities include:

  • Support individuals found eligible for case management services through Intellectual and Developmental Disabilities

  • Assess client needs and risk status through home visits, phone calls

  • Connecting them with resources and meetings with community partners involved in the person's care

  • Develop comprehensive, client-centered care plans for people living in family homes and in comprehensive 24 hour care

  • Monitor services and supports

Successful case managers have the:

  • Knowledge of principles and practices of case management for people impacted by intellectual and/or developmental disabilities

  • Ability to stay on top of the clerical duties so the client work stays up to date

  • Commitment to delivering person-centered care

  • Comfort and competence with using standard technology

Minimum Qualifications/Transferable Skills*:

Education/Training:

  • Bachelor's degree from an accredited college or university with major course work in social work, psychology/sociology, criminal justice or a related field. (Professional work at or near the level of this position can substitute year for year for the educational requirement.)

  • Three (3) years of increasingly responsible research, field investigatory or case management experience as a journey-level social services, court, criminal justice or corrections/protective services professional

  • Must pass a criminal background check
