Case Management Team Lead

Full time, Day, 8am - 5pm

Bel Air, MD

JOB SUMMARY: On a daily basis the Case Manager Team Lead will ensure safe, timely and appropriate provision of services. Coaches team members to problem solve cases to optimize patients' health, reduce unnecessary care and limit healthcare spending. Monitors team member documentation and data collection to ensure complete, accurate and compliant records. Works collaboratively with the Population Health management team, as well as internal and external stakeholders. May interact with patients or customers ranging in age from infant to geriatric. Position requires coverage on holidays and on back- up call system on weekends.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:

Education & Training: Current RN license required OR Current Social work license LBSW, LMSW, LCSW, LCSW-C required. Associate's degree or diploma in nursing OR Master's in Social Work degree required. Bachelors of Science in Nursing preferred.

Work Orientation & Experience: Two years of experience in Case Management and understanding of Utilization Management preferably in an acute care setting required.

Skills & Abilities:

Demonstrate skill in a) clinical case management; b) data analysis; c) effective critical thinking; d) written and oral communications; and e) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult). Demonstrate knowledge of a) acute care criteria and b) case management principles. Demonstrate ability to a) communicate and collaborate effectively with both internal and external customers (colleagues, Medical Staff, liaisons, and patient/family); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, and ethical standards; d) prioritize work assignments and manage time effectively to complete duties; and e) be available to adapt hours to meet the needs of all customers.

JOB TITLE: Case Manager, Team Lead

JOB TASK LIST

1.Assist with Human Resources functions:

  • Input on team member performance appraisals based on objective criteria & data.

  • Active member of the interviewing and selection process for vacancies.

2.Assist manager with coordinating daily team assignments based on review and providing coverage and support in areas of need.

3.Team Development:

  • Serves as a role model for the Care Coordination team across the continuum and helps promote opportunities for the team to understand the vision of Population Health.

  • Lead team huddles and DASH

  • Coordinate orientation of new team members

  • Coordinate education opportunities both internally and externally for the staff

4.Care Coordination:

  • Round daily with team members to identify coordination of care issues and length of stay patients > 4 days.

  • Assisting and monitoring team members to problem solve cases to optimize patients' health, reduce unnecessary care and limit healthcare spending.

  • Coordinate case conferences with appropriate entities to identify patient's health care needs and minimization hospital utilization.

5.Performance Improvement:

  • Offer insight in setting annual goals and objectives for the Clinical Resource Management.

  • Participates in performance improvement activities to improve service and interdepartmental communication in all aspects of the PI plan to team members.

  • Provide input on potential process problems preventing achievement of goals

6.Documentation:

  • Monitor and track timeliness, accuracy, completeness and compliance of team members' documentation in Meditech & MIDAS.

  • Identify inconsistencies in team members' input of data

  • Assist in the development and implementation of automated reports/worksheets.
    7.Discharge Planning:

  • Assess patients, identify discharge needs and coordinate discharge plan when required

  • Assist in implementation of long term post-acute plan for patients

8.Assists in educating the CRM team on the policies and procedures that are upheld by the

Population Health Inpatient Case Management Department. Assists in ensuring same are followed to promote accuracy and consistency among team members.

9.Coordinate orientation of new team members. Assist in updating and monitoring team member orientation material/binder.



  1. Participate and help coordinate education opportunities both internally and externally for the staff.

  2. Proactively serves as a resource to physicians and team members to provide guidance and


understanding of the interdepartmental workings across the continuum of care; i.e. Comprehensive CARE Center and the Clinical Resource Management departments.



  1. Establish an effective and appropriate means of communicating and collaborating with providers, team members, payers and ancillary services to ensure safe and efficient services.

  2. Assist with special projects and other duties as assigned.
