The primary purpose of the Supervisor, Case Management position is to: Oversee and provide Case Management Services in the inpatient and outpatient settings 7 days a week as assigned.
Responsible for the quality of Case Management services delivered, competency and performance of Case Managers, Case Management Assistants, Case Management Resource Specialists, Utilization Review Specialists and Support as assigned. Designs, maintains and improves systems to support departmental functions across the continuum, internally/externally.
Ideal candidate will have a BSN, 5 years or more of clinical nursing experience, two years of supervisory experience and will have case management experience.
Minimum - $90,000 $112,500 - $135,000
Demonstrates effective leadership skills by holding direct reports accountable for their behaviors, interpersonal skills, performance, decision making and adherence to policy and procedure. Initiates and facilitates effective open two-way communication and resolution of operational issues that support case management service area goals and objectives.
Responsible for and authorized to lead work groups, resolve routine issues, and create a positive, productive and fiscally responsible work group. Provides timely feedback, coaching and corrective. Evaluates staff performance through rounding on assigned work areas and provides assistance and guidance as appropriate
Promotes culture of service excellence and quality by implementing and accepting accountability of the customer service expectations: provide excellent service to patients/customers; build strong relationships and team cohesiveness; focus on quality and positive solutions. Ensures increased visibility of case management staff on assigned geographic units.
Quality and Compliance:
Conducts employee and customer rounding as scheduled; Submits associated reports and action plans to Associate Director and Director. Maintains knowledge and support of departmental wide compliance associated with regulatory and payor requirements; develops appropriate corrective action plans to ensure and demonstrate compliance.
Ensures that all requested information/reports from staff are submitted according to requested timeline/deadline. Monitors attendance of staff and initiates progressive disciplinary action as required. Provides direction and monitors staff efforts to ensure product and services consistently meet the needs and requirements of patients, customers and coworkers.
Required: Bachelor's degree in Nursing.
Preferred: Master's degree.
Required: Five years of clinical nursing experience in a large academic healthcare institution to include two years' experience in a supervisory/management role.
Required: Current State of Texas Professional Nursing license (RN).
