Coordinates care of an assigned caseload of patients through collaboration with the case managers and social workers. Facilitates onsite communication for virtual case managers to improve departmental outcomes by acting as a liaison between the Virtual Case Manager, patient/representative and Baptist Health staff as on-site presence to follow up with patients, family/representatives and staff on case management issues. Works with the interdisciplinary team, patients and their representatives to facilitate transitions from the hospital to the community.
Degree in a healthcare-related field degree required, preferably as LPN or BSW.
Two (2) years of direct patient experience in a hospital setting preferred.
If LPN, Alabama LPN license required
Proficiency in medical terminology.
Understanding of basic operations, services and activities related to direct patient care in a hospital setting.
Effective communication and interpersonal skills.
Ability to establish and maintain cooperative working relationships in support of patient care.
Ability to work independently with minimal supervision.
Must be able to function in high-stress environment subject to frequent interruptions and changing priorities.
Knowledgeable in use of appropriate infection control or isolation techniques when necessary.
