BASIC FUNCTION:

This position is responsible for performing concurrent review in accordance with accepted department criteria; performing discharge planning and identifying alternate treatment programs that provide medically necessary potentially cost effective services; consulting with physicians, providers, members, and other resources, as appropriate, to assess, plan, facilitate implementation, coordinate, monitor, and evaluate options and services required to meet an individual's health needs, using communication and available resources to promote quality, cost effective outcomes; serving as liaison to physicians, and members; serving as preceptor for less experienced staff.

JOB REQUIREMENTS:

  • Registered Nurse (RN) with unrestricted license in state.

  • 3 years clinical practice experience.

  • 1 year experience in health insurance/managed care.

  • PC and database experience.

  • Verbal and written communication skills.

PREFERRED JOB REQUIREMENTS:

  • Experience in Disease Management.

  • Experience in managing complex or catastrophic cases.

  • Working toward CM certification or Advanced degree.

  • Knowledge of medical management policies and procedures

LEVEL DESCRIPTORS This job family consists of four levels which are distinguished by the level of complexity and diversity of the specific job assignments, the extent or responsibility assigned for each level, the expertise required for the completion of duties assigned, and the responsibility assigned for providing leadership to others. Level I: This is the basic level where employees are assigned responsibilities involving beginning level casework activities in a training status to build skills in providing case management services to consumers. In this role they will be assigned tasks which involve managing a caseload of moderate complexity and providing needed services; acting as the consumer's QMRP, and monitoring all aspects of the consumer's program and service delivery; and serving as the individual's advocate. Additional responsibilities involve ensuring the development, implementation, monitoring/evaluation and modification of the Individual Habilitation Plan (IHP); including the development of individualized treatment plans; developing plans of care and reviewing the IHP/Plan of Care, notifying the consumer and the family/guardian/significant others with appropriate community resources; completing all necessary transition activities. Level II: At this level employees are assigned responsibilities at the full performance level in managing a caseload of substantial size and complexity consisting of all disability areas; acting as advocate for the individual to ensure community presence and participation. Supporting the individual and family/guardian/significant others to make life decisions which lead to independence and interdependence; serving as the team leader of the Interdisciplinary Team (IDT); ensuring the development, implementation, monitoring/evaluation and modification of the IHP for desired outcome and developing the plan of care; including the development of individualized treatment plans. Additional responsibilities include coordinating guardianship procedures for adult consumers who do not have legal guardians; providing individual and family counseling to consumers and their families with social and/or emotional problems; assisting Level I case managers by furnishing information concerning case management practices, standards, rules and regulations and community resources, providers and programs; serving as resource individual for Level I case managers in providing information concerning specialized programs, services and treatments; and participating on committees at both local and state levels to formulate policies and procedures and to promote community awareness. MINIMUM QUALIFICATIONS Level I: Education and Experience requirements at this level consist of a bachelor's degree in a human services field* and one (1) year of experience working directly with persons with intellectual disabilities; or possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with persons with intellectual disabilities.*For purposes of the Case Manager job family "a bachelor's degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
