Case Management Assistant

Job Description

Performs diversified clerical duties supporting department case management activities. Coordinates patient placement with post-acute care facilities and/or services.

Job Responsibility

• Supports case managers and social workers on assigned units with administrative aspects of transition planning activities. Gathers and processes required documentation to facilitate placement.
  
  Communicates with post-acute care facilities to initiate patient transfers. Obtains appropriate insurance authorization for post-acute care services. Arranges for patient transportation upon hospital discharge, as required.
  
  May attend interdisciplinary patient rounds. Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.

Job Qualification

• High School Diploma or equivalent required.

• 1-3 years of relevant experience, required.
