The Long-Term Services and Supports (LTSS) service area of the Health and Human Services department, has an opening for a Case Management Assistant (CMA) position. This position will work with a team that serves as a liaison between Hennepin County and contracted case management agencies that administer Medicaid waiver and state programs in addition to serving seniors, serve people with brain injury, mental health needs, complex medical needs, physical, developmental, and intellectual disabilities.

Location and hours:
This position is remote. Remote workers may be required to come on-site 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours will be Monday through Friday, 8 a.m.

In this position, you will:
Work with and maintain an understanding of case management processes used in the operated case management (OCM) and contracted case management coordination (CCMC) team areas.
Contact providers to gather information, make referrals, and assist individuals in obtaining social services.
Proficiently navigate county and state database systems, manage data in Excel and other Microsoft applications, verify Department of Human Services (DHS) licensing or other reporting, and manage systems and assist case managers and/or supervisors in keeping system information up to date.
Assist case managers and supervisors with audit follow up tasks and obtain necessary paperwork for waiver programs including signature pages, ICD-10 codes, etc.
Support caseload transfers and act as a backup for the transfer team.
Assist in monitoring Medical Assistance for individuals served.
Use continuous process improvement approach and work collaboratively with others.
Participate in team's effort to ensure processes/protocols are followed to ensure program compliance and progress toward meeting any targeted outcomes.
Meet with clients as necessary to obtain necessary paperwork for participant programs, arrange transportation and/or transport participants to appointments as necessary.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Nice to have:
Experience:
Using the Social Service Information System (SSIS), Electronic Case Files (ECF), and/or the Medicaid Management Information System (MMIS).
Working with medical assistance, waiver programs, and/or social services.
Using a computer to input, retrieve, and modify information, and performing data entry from paper/electronic documents and forms into a computer system.
Knowledge of community resources and social services available to clients.Effective interpersonal and writing skills.Ability to:
Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
Make sound, independent judgment.
Work both independently and collaboratively as an effective team member.
Establish and maintain professional working relationships with clients, staff, and the public.
