Baptist Medical Center is looking to add a Case Coordinator in our Outpatient Behavioral Health unit. This will be a Full Time Days position located at Baptist Jacksonville (downtown campus)

The position will primarily be in office to assure collaborative relationships with the primary care physicians have been established. The Case Coordinator (CC) is a core member of the collaborative care team, which includes, but is not limited to, various mental health professionals, psychiatrists, and primary care physicians. Travel between and some remote work may be required.

As a Case Coordinator, you will be responsible for:

• The Case Coordinator assists the patients of Outpatient Behavioral Health and their families in obtaining needed services within the Baptist Health system and across other community entities, in accordance with a care plan developed with the patients' providers.

• The CC will employ a biopsychosocial approach to care for the "whole patient".

• The CC focuses on specific mental health patient populations; i.e. mild to moderate symptomatology of diagnoses such as depression, anxiety, and ADHD.

• The CC will provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

• The CC will track patients in a registry, utilize measurement-based practices and treatments, document progress, and facilitate referrals when needed.
