Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement. As the area’s only PACE program, ElderONE represents the future of senior health care.

This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety. ElderONE SCHEDULE: Monday-Friday ATTRIBUTES Compassionate, patient focused and dependable professional Critical thinking skills, decisive judgment and the ability to work with minimal supervision Exceptional documentation and communication skills Ability to read, write and speak English, ability to follow verbal and written instructions RESPONSIBILITIES Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands Communication.

Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly Documentation. Accurately and completely document client care on activity sheet Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements EDUCATION:

HS (Required) LICENSES / CERTIFICATIONS: CNA - Certified Nursing Assistant

• New York State Education Department (NYSED), HHA - Home Health Aide certification

• New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work

• Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

PAY RANGE: $17.60 - $19.50 CITY: Rochester POSTAL CODE: 14617 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.

Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
