The Caregiver (PRN) provides personal care and incidental Activities of Daily Living to RCAC, CBRF and/or Memory Care residents of Capri Communities LLC, with adherence to organizational and regulatory requirements.


  • Must be available to work every other weekend.

  • Adheres to the written Resident Care Plan to:

  • Assist RCAC, CBRF and/or Memory Care residents in maintaining their greatest independence with activities of daily living such as bathing, grooming, toileting and nutritional needs. Also provides assistance with preparing meals, laundry and maintaining a clean, safe physical environment.

  • Assist residents with mobility and transfers

  • Assist residents with medical administration and maintains related reporting requirements.

  • Perform delegated tasks as instructed by the RN, Nurse Manager, Executive Director, Assistant Executive Director, Lead Caregiver or Lifestyle Enrichment Specialist.

  • Perform acceptable standard healthcare practices.

  • Assists with housekeeping duties in resident or common areas (vacuuming, dusting, sweeping, cleaning, etc.) as directed.

  • Leads or assists with activities for residents as directed, impromptu and scheduled.

  • Observes and reports resident's condition and safety hazards to RN, Nurse Manager, Assistant Executive Director or Executive Director in a timely manner.

  • Documents observations and activity participation in an accurate, complete and timely manner.

  • Maintains excellent work attendance; is punctual for work shift.

  • Possesses organizational skills necessary to carry out shift duties and to complete assigned tasks while promoting residents emotional well-being and ability.

  • Protects residents' health information according to HIPPA policies and procedures and federal and state law and regulations.

  • Maintains professional appearances, attitude, and demeanor while on duty.

  • Maintains knowledge of State Regulations and updates for DHS 83 and 89 (as appropriate for RCAC, CBRF and/or Memory Care services).

  • Attends all mandatory staff meetings and in-service training opportunities.

  • Performs other miscellaneous tasks as assigned.


Must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  • High School education, plus:

  • Six months of experience as a personal aide, Caregiver or equivalent life experience

  • Completion of CBRF and/or CNA certifications

  • Reliability and Flexibility in Scheduling, must demonstrate excellent attendance; must be able to work holidays and weekends and cover as required by resident needs.

  • Ability to demonstrate patience, understanding, tolerance and emotional/physical ability and experience to meet the daily needs of our residents.

  • Government Compliance: Demonstrates knowledge of and complies with all building regulations and DHS requirements. Maintains all training requirements as directed by DHS 83 and/or 89. Ensure that all fair housing standards are met at all times. Comply with OSHA, Workers Compensation, and any other reporting requirements

  • Resident Satisfaction: Provide, promote, and require high-quality resident relations. Review and/or resolve complaints, and alert Nurse Manager of any unusual activities or problems.

  • Quality, Safety, and Physical Condition Administer care that meets or exceeds company safety and physical condition standards. Cooperate in maintaining safety and security of residents and employees. Ensure that residents and staff are informed about safety issues, regulations, and requirements necessary. Ensure the safe and appropriate execution of food preparation and use of equipment. Enforce compliance with regard to safety and health issues as well as appropriate food standards.

Part time work means the person hired will be scheduled to work at least 40-hours in a two week pay period. Work Schedule Second Shift: 3:00 PM to 11:00 PM or Third Shift: 11:00 PM to 7:30 AM including weekends and holidays Must be available to work other shifts as needed Example of Duties Under supervision to assist in-patient psychiatric patients by providing basic care services and maintaining a safe and secure environment; and to perform other duties as required. * Monitors patient behaviors and activities at all times and responds to all patient needs and concerns. * Records observations regarding the patients' behavior, mental status, and care; reports observations to the primary nurse. * Under the guidance of licensed staff, assists in restraining physically aggressive patients to protect them from harming themselves, other patients, or staff. * Assists nursing staff with new admissions by taking vital signs, orienting patient to the unit, and checking patient belongings for contraband items. * Provides daily nursing assistance by monitoring vitals, recording intake, output, and weight, and by assisting with activities of daily living. * Prepares patients for meals, picks up and portions meals, checks meals for correctness, assists with feeding as necessary, and cleans up following meals. * Directs or assists patients with personal hygiene care, including such things as bathing, shaving, and hair care. * Provides clean linen for bed changes and assists patient in keeping room clean. * Transports patients to medical appointments as needed * Plans and conducts daily patient educational groups and assists with other group activities as assigned by charge nurse. * Plans and implements recreational activities on the weekends and during evening hours. * Monitors visitation activities including checking in visitors, checking for contraband, and monitoring behavior during visits. * Responsible for keeping work area clean and in order by keeping floors dry, storerooms tidy, corridors clear, disposing of waste properly, and washing equipment regularly. * Participates in in-service training programs to remain current in the mental health field. * Transports and directs patients and visitors to safety in emergencies such as fire or severe weather conditions. * Performs other duties as required. QUALIFICATIONS Essential Knowledge and Abilities * Working knowledge of safe work practices and procedures in an in-patient mental health facility. * Working knowledge of general types of mental illnesses, treatments, and medications. * Working knowledge of confidentiality statutes and requirements including HIPAA. * Working knowledge of computers and departmental program software and internet access. * Some knowledge of the effect and interaction of substance abuse on patients with mental illness. * Some knowledge of medical terminology to accurately reflect patient conditions in daily charting. * respond to the patient's physical and emotional needs in varied settings and situations. * Ability to analyze patient functioning and develop an understanding of patient problems and needs. * Ability to assume and carry out considerable responsibility within established guidelines and with limited supervision. * Ability to maintain accurate records of patient observations, interactions, and care administered. * Ability to restrain physically aggressive patients and to de-escalate psychologically or emotionally agitated patients. * Ability to maintain self-control and respond appropriately in a crisis situation. * Ability to act as a co-leader in planning, organizing, and implementing structured group activities and recreational activities. * Ability to communicate effectively both verbally and in writing with clients, families, staff, and medical, legal, and law enforcement personnel. * Ability to establish and maintain effective working relationships. * Ability to utilize electronic health record programs.
