Full-time Positions

Overnights 10:45 pm - 7:00 am
1.00 shift differential paid for overnight shifts

POSITION PURPOSE

The Caregiver position exists as part of the Assisted Living team to assist the residents of Golden West's assisted living community with daily living activities with the goal of enhancing each resident's quality of life. The caregiver is responsible for providing personal assistance to 14 elderly residents. Duties vary depending on shift worked and may include meal service, housekeeping, medication supervision, bathing assistance, resident socialization, care planning, night checks and emergency response.

REQUIREMENTS

  • Read, write and speak English.

  • Pass the State-approved Medication Administration course.

  • Excellent interpersonal and communication skills with seniors, family members and health care professionals in person and over the telephone.

  • Capable of pushing up to 40 lbs and lifting up to 20 lbs as well as bending, kneeling and reaching. Must be able to be on feet, walking and standing for at least 8 hours a day and be able to sit in a chair for at least 1 continuous hour.

  • Ability to move swiftly throughout all areas of the community and property, including stairwells and elevators.

  • Capable of handling interruptions and completing tasks after an interruption.

  • Demonstrate attention to detail.

  • Maintain and project a professional, pleasant, and helpful attitude toward others at all times.

PRINCIPAL ACCOUNTABILITIES

  • Work as a productive member of the assisted living team, maintaining a positive and professional attitude at all times.

  • Be prepared to begin work, complete with appropriate dress, by the scheduled start time of each shift.

  • Maintain an acceptable attendance record. Record all hours worked on time card and take breaks as per assisted living and Golden West policies and procedures.

  • Be familiar with and follow all Golden West personnel and safety policies.

  • Work as a team with all scheduled caregivers to complete the following tasks, as per department procedures and scheduled times, necessary to provide a quality living environment for the residents:

  • Administer medications to residents participating in the medication program.

  • Assist residents in getting ready for the day as indicated in the care plans, i.e. fill portable oxygen tanks, assist with minor dressing needs.

  • Assist residents with daily living activities, i.e. provide reminders to ensure that they participate in meals and activities of their choice.

  • Document all significant resident changes and unusual occurrences in the daily logs. Alert appropriate personnel when a resident has a change in condition.

  • Respond to emergencies according to the procedures outlined in the Caregiver manual.

  • Participate in care planning and family conferences for residents on assigned floor as needed.

  • Assist with meal service

  • Launder residents' personal clothing.

  • Provide bathing assistance as needed.

  • Attend Daily Shift Report and other meeting and in-services as scheduled.

  • All other duties as assigned by supervisor.

POSITION PURPOSE The Lead Caregiver position exists as part of the Mezzanine team to assist the Coordinator and Resident Care Manager with daily operational tasks to help ensure that residents are receiving the highest quality of care. The Lead Caregiver is responsible for providing support to the Coordinator and Manager as well as guidance to caregivers, families and vendors. Duties vary depending on shift worked and the needs of the department, but may include assisting with the medication program, resident services, administrative tasks, training a new caregiver or covering an open caregiver shift. PRINCIPAL ACCOUNTABILITIES * Work as a productive member of the Mezzanine team, maintaining a positive and professional attitude at all times. * Be prepared to begin work, complete with appropriate dress, by the scheduled start time of each shift. * Maintain an acceptable attendance record. Clock in/out properly and take breaks as per Mezzanine and Golden West policies and procedures. * Be familiar with and follow all Golden West personnel and safety policies. * Responsible for covering open caregiver shifts on scheduled days. Must be flexible with schedule and available to cover day, evening and overnight shifts, depending on the staffing needs of the department. * Assist with medication program * Stock medications on floors as directed by Resident Care Manager * Complete tracking of arrival of ordered medications * Call pharmacy to follow up on delivery issues * Process new physician orders as directed by Resident Care Manager * Submit new orders to pharmacy * Update Medication Administration Record (MAR) * Check cycle meds as directed by Resident Care Manager * Complete weekly cart audits for medication levels * Complete audits of all medication carts to check for medications that are running low or close to expiration date. Audits will include PRN medications, bottles, creams, drops, etc. * Report issues to Resident Care Manager * Assist Resident Care Manager with destroying medications * Weekly audit of medication orders * Audit each floor to ensure a current physician's order is in the MAR for each medication * Check that the most recent order is on top * Ensure orders match the medications in the cart * Bring any discrepancies promptly to the attention of the Resident Care Manager * Conduct weekly audits to ensure residents' oxygen tanks are stored properly in the apartment * Audit MARs for holes * Follow up with caregivers on routine missing initials * romptly notify Resident Care Manager of concerns (frequent holes, recurring patterns, possible meds not administered) * Assist with resident services * Inventory and order incontinent supplies for residents * Schedule transportation arrangements for residents as directed by Coordinator * Meet with residents to discuss any questions/concerns as directed by Coordinator * Follow up with outside service providers as directed by Coordinator * Audit Task Administration Record (TAR) for holes * Follow up with caregivers on routine missing initials * Promptly notify Coordinator of issues (frequent holes, recurring patterns, service not provided) * Assist with administrative tasks * Audit office supplies, first aid kit and med cart supply levels * Restock as needed * Alert Coordinator when supplies are low * Ensure adequate supply of all documentation forms which include: PRN sheets, narcotic count sheets, meal trackers, medication incident reports, narcotic release forms, sign in/out sheets, observation forms, incident reports, Story of My Fall reports, etc. Re-stock forms on the floors as needed. * Complete filing tasks as needed * Provide caregiver support * Cover open shifts during scheduled work days, as needed * Provide guidance and support to caregivers with emergencies and difficult situations * Answer caregiver questions regarding procedures/tasks * Provide coverage on the floors for caregiver breaks * Assist in training new caregivers * All other duties as assigned by supervisor.
