Essential Duties:

  • Serve as a resource and support to schools as they assist students with disabilities served under the Individuals with Disabilities Act (IDEA) in the career, technical, and agricultural education clusters, and coordinate the services needed for the learner to successfully meet transition goals.

  • Support teachers to ensure the implementation of current best practices in the transition curriculum.

  • Provide support to special educators regarding transition assessment materials (including technology) and strategies.

  • Assist teachers with providing transition assessments to secondary ESE students, which includes Career & Technical Testing and test interpretation for students.

  • Identify and support the use of research-based practices related to secondary transition.

  • Provide support to teachers and community businesses to maintain work sites for CBVI and WBL.

  • Assist case managers with coordinating student's course of study with short and long-range career goals through an Individualized Education Plan (IEP).

  • Attend individual student transition meetings as required.

  • Find alternative training/educational programs for ESE students who may be considering dropping out of school.

  • Gather employability-related information for students that will assist and empower them in making career decisions.

  • Solicit outside community businesses, stakeholders, and agencies to speak on available resources to the Interagency Team Council or other appropriate groups.

  • Coordinate and support the services offered to the school and students through the Vocational Rehabilitation Agency.

  • Work as a liaison with transition-related agencies and/or programs outside of the district and the schools they support.

  • Provide information to teachers, parents, and students regarding transition planning and available programs.

  • Organize orientations at secondary schools regarding transition agency services.

  • Assist students and parents with applications for programs such as: Division of Vocational Rehabilitation Services, Medicaid Waiver, etc.

  • Provide on-going special topic seminars for teachers, parents, and students on topics related to post-secondary transition such as: SSI, State Medicaid Waiver programs.

  • Facilitate presentations to school and community groups related to post-secondary transition requirements and activities.

  • Facilitate professional development upon request related to any topics within the job description.

  • Perform other duties as assigned.

Qualifications:

  • Minimum of a T-4 Georgia Educator Certificate in Special Education or related field; T-5 Educator Certificate is preferred.

  • Minimum of five years of successful certified level experience in education, preferred.

  • Knowledge of transition activities and post-secondary opportunities for students with disabilities.

  • Knowledge of federal, state, and local rules and regulations.

  • Skilled at collaborative decision-making for individual students and staff growth.

  • Skilled at working with a diverse group of students with disabilities.

  • Exceptional integrity, commitment to confidentiality, excellent character, and strong professional reputation.

  • Ability to abide by the Georgia Professional Standards Code of Ethics for Educators.

  • Must be regularly, predictably, and reliably at work.

  • Ability to be present at the physical worksite.

  • Must perform tasks involving prolonged periods of standing, walking, and sitting.

  • Must perform routine physical activities such as bending and lifting/pushing/pulling up to 50 pounds.

  • Vision, hearing, written, and verbal communications are essential factors in performing required tasks.
