Under direction and supervision of an RN and administration, the Care Transitions Tele-Medical Assistant follows all policies, procedures and professional guidelines of TGH. Responsible for coordination and processing of all patient referrals for specialty services, communication with patients on a timely basis for all scheduling requirements and providing administrative and clerical support within Ambulatory Transitional Care that may include telephone support, filing, typing, coordinating schedules, distributing correspondence, materials management, data entry, simple data analyses, and rudimentary reporting.

All within established legal guidelines and maintaining confidentiality at all times. Essential Functions: Consistently exhibits behavior and communication skills that demonstrate our company's commitment to superior customer service, including quality, care and concern with each and every internal and external customer. Uses, protects, and discloses our company's patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.

Processes all requests for information following established legal guidelines. Supports the preparation for audits and data requests.

Assists in the preparation of reports. Conducts patient interviews, answers, screens and transfers telephone calls. Maintains schedules by coordinating with appropriate parties. Organizes and distributes correspondence, reports, memos, etc.

Conduct outreach calls and community-based outreach efforts to enroll and schedule patients. Serves as liaison between the Inpatient Care Coordination team and Ambulatory clinics.

Monitors remote patient data and assists with manual data entry. Maintains database of information for area of responsibility. Analyses data and compiles statistics.

Using remote telemonitoring, review, record and conduct any necessary vital signs including blood pressure, oxygen saturation, blood glucose reading, weight, height, temperature, and any other assigned items. Provides follow-up intervention and coordination with specialty clinics, based on established clinical guidelines. Summons appropriate staff if telemonitoring patient requires in person assistance.

Work with patients and providers to schedule telehealth encounters. Facilitate telehealth consultations by pre-visit chart review or planning Provides medical education, health promotion and discharge instructions to patients per order of the medical clinician. Complies with operational procedures & collaborates across disciplines to facilitate warm hand-offs.

Assist the Medical Providers in obtaining any necessary clinical patient information, such as lab results, diagnostic tests performed, hospital summaries imaging results, discharge summaries, and current medications. Job duties are in accordance with the mission, vision and values of Tampa General Hospital.

High School diploma or GED. Medical Assistant Technician Certificate required. Graduate of a Medical Assistant program or three years of experience required.

Will be required to complete TGH - CMT training within 1st 6 months of employment.

Preferred: Previous medical office experience with referral processing for HMO plans is preferred. Prior Telemonitoring Experience. Bi-Lingual (Spanish)
