Responsibilities

Brookdale Hospice is looking for a Care Transition Manager/Hospice Care Liaison to join our team- LPN/LVN or LSW with a passion for educating and working with Residents and their Physicians! In this role you will be a liaison between our Brookdale communities as well as non- Brookdale communities and Brookdale Hospice, families and physicians to help make the transition to hospice a smooth one.

Your responsibilities:

  • Facilitate a consistent service delivery of the Brookdale Hospice Services continuum including highest quality care for the patient and family

  • Participate in but not limited to identification of clinical needs with residents & the coordination of collaborative Care for Hospice Services while additionally accountable for completing the referral process including communicating to the patient, the family, the staff and other professionals as appropriate

  • Participate in regular meetings with Executive Directors and Area Managers to review customer experience, quality of care delivery, and program growth

  • Serve as an educational resource to the community regarding healthcare services

Qualifications

Education and ExperienceCare Transition Manager/Care Coordinator Hospice LPN, LVN or LSW preferred. Minimum two years of experience in healthcare with experience participating in Collaborative Care (or similar) meetings and influencing customers preferred.

Certifications, Licenses, and Other Special RequirementsLPN/LVN or LSW preferred.

Management/Decision MakingUses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

Knowledge and SkillsHas a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Proficiency in Microsoft Office and Windows. Ability to work holidays and weekends as operation demands. Ability to maintain the highest standards of ethical, professional, and clinical judgment. Ability to address staff, residents, and visitors in a courteous and friendly manner. Effective verbal and written communication skills required. Must be willing and able to communicate with residents of various levels of functioning and various types of personalities. Willingness to communicate perceived physical and emotional needs of residents to other skilled staff to assist with addressing their care needs.

This resource will collaborate with other operational resources to provide management with recurring and ad-hoc operational metrics, as well as aid in extract, transformation, and load of data to development servers. Primary Responsibilities: * Build and manage operational reports that are used to support the business on a daily basis (Tableau and Excel) * Maintain reporting repository and definitions related to the business * Identify and/or create relevant database tables/fields needed to source appropriate data * Create development ETL processes, ad-hoc and production candidate, to help facilitate answering operational questions across multiple systems/platforms (SSIS) * Ensure customers have appropriate access to reports * Test and verify the accuracy of incoming or received data and/or coding prior to generating automated reports * Provide support to internal customers on issues that arise when creating reports * Facilitate monthly billing and record maintenance tasks * Present results and reports to internal customers (e.g. new reports, PowerPoint presentations, etc.) * Review ad hoc reporting requests in order to determine whether a standard reporting solution can be created * Review manual report packages to identify opportunities to automate reporting and/or eliminate redundancies Required Qualifications: * Ability to build complex queries, functions, and procedures using MSSQL (Transact SQL) * Reporting or database experience either from an post-grad academic or professional setting * Experience using Tableau including knowing how to build and maintain a Tableau dashboard * Experience using Excel including pivot tables, filter, sort, edit, manipulate, create formulas Preferred Qualifications: * Bachelor's degree Economics, MIS, Health Administration, Mathematics or related field * SSIS and/or Equivalent ETL experience * Healthcare/Insurance experience Careers with Optum.
