Swedish Medical Group is looking for a Care Transition Coach (1.0 FTE, Day Shift) to work for the Collaborative Health Action Team at the Bank of America Building in Seattle, WA.
The Care Transition Coach is reponsible for coordinating the changing health and service needs of the post-acute patient by maintaining contact with the patient, understanding the patient disease process, coordinating post-acute appointments, providing clinical education and coordinating overall care with primary care physicians.
In this position you will:
Conduct effective telephonic visits that promote patient self-care management within the time frame identified by guidelines
Coordinate services to ensure patients' timely follow up with Primary Care Provider (PCP) within established time frames
Collaborate with Case Manager RN, PCP and community resources regarding individual case issues and changes in patient condition
Required qualifications for this position include:
Graduate of accredited School of Practical Nursing
Current Washington State LPN license
Current BLS certification required or to be obtained at department expense within one month of hire
Preferred qualifications for this position include:
Minimum of three years current experience in a health care environment with home health case management experience
Knowledge of community resources and the ability to carry out nursing responsibilities in diverse settings under difficult situations
