Full-time, Monday-Friday with rotating Saturdays Sign-Bonus: $3,000 YOUR ROLE: As our Care Team Nurse, you will use a trauma-informed and whole-person approach to address patient wellness, acute illness, chronic disease, disability, and end-of-life needs in collaboration with their assigned primary care provider and care team. In this role, you will educate and motivate patients in areas related to self-care; dispense telephone advice to patients with minor acute problems; support the ongoing care management of patients with stable chronic disease; monitor patients during routine office visits; and perform of a variety of procedures that are within the scope of the professional nursing practice.
