Job Description:

JOB SUMMARY

The Care Team Coordinator functions as an important member of the organization's healthcare team and is responsible for delegated tasks under the direct supervision of a provider. The Care Team Coordinator rooms the patient, completes team documentation and preforms in-between visit functions within the specific team care structure to provide excellent patient care to those we serve.

JOB QUALIFICATIONS

EDUCATION

For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Minimum Required: Graduate of a medical assistant (MA) or license practical nurse (LPN) program.

Preferred/Optional: None

EXPERIENCE

Minimum Required: None

Preferred/Optional: One year experience as a Medical Assistant (MA) or Licensed Practical Nurse (LPN).

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.

Minimum Required: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire and one of the following:

  • Certified Medical Assistant (CMA) certification awarded by the American Association of Medical Assistants (AAMA)
  • Registered Medical Assistant (RMA) certification awarded by the American Medical Technologists (RMA)
  • Licensed Practical Nurse license awarded by the State of Wisconsin

Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.

As a Flex RN Care Manager, you will be responsible for partnering with your fellow colleagues to provide top quality care to our members. They are looking to you in providing assistance when it comes to living their daily lives. What are the primary duties of this RN position? * Partner with a social services care manager also referred to as a Community Resource Coordinator, and collaborate on an ongoing basis regarding the member and their plan. * Perform face-to-face health-related assessments for members upon enrollment and at least every six (6) months thereafter in the member's setting. * Coordinate, schedule and track monthly phone contacts and quarterly face-to-face visits. * Participate in the development and ongoing review and coordination of the member's plan of care. * Take the lead on all health issues for the member. * Contact and coordinate with acute and primary care providers. * Identify and coordinate clinical and functional supports needed for the member's health and safety. * Educate on and promote prevention and wellness and mitigate risk when assessed. * Document activity and complete paperwork as required. This is a FLEX position and must have availability to travel up to 2 hours from the Merrill office to visit members. Travel is eligible for mileage reimbursement.
