This position provides psychosocial case management services for patients needing assistance. The Care Navigator will assist patients and collaborate with other health professionals to ensure patient wellness.

The Care Navigator will function as part of a multidisciplinary health care team working with providers and other clinical staff at Kelsey-Seybold Clinic. Services provided are generally crisis oriented and are geared to assist patients make proper use of the health care system, while improving the outcome of care. The Care Navigator assesses, counsels, and provides psycho-therapeutic treatment, rehabilitation, and care coordination for adults with mental illness and/or addiction.

The Care Navigator will perform comprehensive assessments and treatment of persons whose lives are impacted by cognitive conditions, mental health issues (depression, anxiety, adjustment disorder, Alzheimer's, Dementia), or a declining capacity to function independently completing cognitive and mental health evaluations; delivering cognitive and emotional therapies; completing timely and accurate charting; as well as providing individual therapy for patients. Provide trauma-focused Cognitive Behavioral Therapy (CBT.) The Licensed Clinical Social Worker (LCSW) will provide quality clinical mental health consultation to patients as requested; provide comprehensive diagnostic mental health assessments, interventions, education, referrals and consultations for patients in the primary care setting including: The Care Navigator will ensure that community resources are available to patients and families.

Social workers must maintain case history records and prepare reports. The LCSW may perform home visits, e-visits, video visits and may work at multiple clinic locations. Other duties as assigned.

Master's degree in social work from an accredited university.


EPIC experience preferred but not required



Requires a minimum of 5 years of combined experience in social work and behavioral health required.


Palliative Care, Home health or Oncology experience preferred but not required



  • Licensed by the Texas Board of Social Work Examiners. (LCSW)- A valid Texas driver's license

Utilizes therapeutic communication in daily practice. Maintains a safe a therapeutic environment. Identifies nursing problems as they relate to the individual needs of the patient. Plans individualized patient care utilizing an interdisciplinary approach. Participates in performance improvement activities. Provides care and coordinates participation that fosters the goals of the hospital's mission and vision statement. When acting in charge nurse role, registered nurses are responsible for day-to-day activities to ensure the delivery of quality patient care in a therapeutic environment. Make staff assignments based on patient need and the skill of the staff. Serve as role models and clinical resource for the staff. Respond in a timely manner to requests for information or follow-up with physician, staff or patient issues.
