Role:

Promote effective partnerships among patients, families, nurses, physicians, and other healthcare providers to coordinate care for patients with chronic diseases.

Essential Functions and Responsibilities:

  • Provide oversight over the care management programs of the Clinic and be the point of reference person for all communications with outside entities.

  • Manage weekly care management reports and update data in the EHR as indicated.

  • Review monthly billing reports and invoices and reconcile as indicated.

  • Communicate with inpatient facility care managers to promote effective transitions of care and oversee appointment bookings for follow-up appointments.

  • Arrange for patient follow-up appointments and testing as indicated.

  • Respond to incoming phone calls from patients/family members with questions about our care management programs.

  • Effectively partner with the care management team to mobilize community resources as needed.

  • Ability to gather, analyze and present data for the purpose of reporting on operational and quality metrics.

  • Maintain and review patient records, charts, and other pertinent information.

  • Attend required meetings and participate in committees as requested.

  • Promotes all care management initiatives of the Clinic and educates staff and providers as indicated.

  • Develop and implement additional care management programs as directed by the medical director or administration.

  • Performs related work as required.

Performance Measurements:

  • Maintain a positive and professional attitude and appearance.

  • Respond to inquiries from patients/family in a prompt, professional and complete manner while acting as a patient advocate, responding with empathy and respect to resolve concerns.

  • Arrange for patient testing, referrals and scheduling as directed.

  • Observe all Clinic policies and guidelines.

Qualifications

Knowledge and Skills:

  • Experience: 2-4 years of experience in either a clinical setting providing care coordination or case management services.

  • Education: Current certification as a Licensed Practical Nurse or Registered Nurse.

  • Interpersonal Skills: A significant level of trust and diplomacy is required, in addition to normal courtesy and tact. Work involves extensive personal contact with others and/or is usually of a personal or sensitive nature. Work may involve motivating or influencing others. Outside contacts become important and fostering sound relationships with other entities becomes necessary.

  • Other Skills: Possess outstanding verbal and written communication skills. Strong attention to detail and the ability to work effectively with rapidly changing priorities. Requires full range of body motion including manual and finger dexterity and eye-hand coordination. Requires sitting or standing for extensive periods of time. Occasionally lifts and carries items weighing up to 50 lbs. Requires corrected vision and hearing to normal range.
