Key Responsibilities:

  • Develop and implement care management plans that integrate physical and behavioral health needs, ensuring continuity of care and client empowerment.

  • Assist clients in accessing health and disability benefits, securing housing, transportation, education, employment, and community resources.

  • Provide preventive interventions and education on topics such as smoking cessation, nutrition, exercise, and stress management.

  • Offer crisis intervention services, collaborating closely with management and therapists to ensure client safety and develop effective safety plans.

Qualifications

Qualifications:

  • Meet the ODMHSAS criteria for Certified Behavioral Health Case Management II (CM II) or obtain certification within 60 days of hire.

  • Minimum one year in a mental health-related field or healthcare setting preferred.

  • Proficiency in MS Word and general computer skills; excellent verbal and written communication skills; ability to work effectively in a multidisciplinary team.

  • Valid Oklahoma driver's license, current automobile liability insurance, and CPR/First Aid certifications required.
