Care Navigator/Health Coach
HSS Lifestyle Medicine Program
Overview:
The Care Navigator/Health Coach for the HSS Lifestyle Medicine Program will serve as a member of the interprofessional care team, providing health coaching services and coordinating patient care from start to finish of the program. This role combines the expertise of clinical care navigation with health coaching to enable patients to develop, implement, and adhere to personal wellness plans that may include:
Introducing mindful thinking and work to build confidence
Defining a higher purpose for improving health
Discovering innate desires and drivers for health
Addressing emotional and physical health together
Enabling patients to create and maintain suitable health plans
Setting realistic, timely, and achievable goals
Helping to problem solve obstacles, recognizing them as opportunities for growth
Providing support for achieving better health
Educating patients and encouraging adherence to their plans of care
Evaluating patients for program discharge and follow-up
This Care Navigator/Health Coach will apply clinical knowledge and expertise to ensure appropriate entry of records and patient measures necessary for monitoring patient progress. The role requires collaboration with an interprofessional care team, including physicians, advanced practice providers, physical therapists, registered dietitians, exercise physiologists, counselors, and program administrators to facilitate and coordinate patient-specific, evidence-based care.
Primarily, the Care Navigator/Health Coach engages in virtual follow-ups, guiding patients throughout their treatment in the Lifestyle Medicine Program. The role involves personalized patient navigation and occasional in-person interactions or group sessions, based on program dynamics and patient requirements. This position benefits from a hybrid work setting with potential in-person office space at various HSS locations such as the Pavilion, East River Building, and Midtown, with specific details to be finalized. Flexibility to work from home is available, dependent on productivity. Regular collaboration with interprofessional team members and active participation in administrative meetings are required to maintain effective patient care coordination.
Key Responsibilities
The Care Navigator/Health Coach plays a crucial role in advancing patient health outcomes through the following responsibilities:
Patient Management
Conduct 1:1 virtual follow-ups with patients to guide and support their care plans within the Lifestyle Medicine Program
Occasionally host group sessions or in-person follow-ups as needed, depending on patient load and program dynamics
Develop and update patient curriculum and educational materials tailored to individual patient needs
Serve as a primary point of contact for patient and staff inquiries, providing consistent clinical and navigational support
Identify and support patients at risk for poor adherence, coordinating with the program's scheduling and integration coordinator to adjust service needs and ensure continuous care
Clinical Team Coordination
Facilitate communication and collaboration among interprofessional team members to ensure comprehensive, patient-specific care
Participate in and actively contribute to monthly interprofessional team meetings, sharing insights from patient interactions to inform care strategies
Administrative Management
Manage and coordinate the scheduling of patient interactions, ensuring flexibility and responsiveness to patient needs
Maintain detailed documentation in the electronic medical record for all patient interactions to track progress and inform care decisions
Communicate effectively with interprofessional team members, staff, and administrators to ensure coordinated and timely patient care
Provide ongoing insight into the care management process, working closely with administration to address barriers and improve patient outcomes
Uphold clear and effective communication with all relevant program stakeholders to foster collaboration and program success
Qualifications
Required Qualifications/Skills:
Must possess a clinical background in fields such as physical therapy (PT), nursing (RN), social work (LCSW), or similar professions, demonstrating a comprehensive understanding of patient care dynamics
Required to hold the appropriate professional certification for their specific clinical field, ensuring they meet all regulatory and practice standards
Previous healthcare work experience
Ability to prioritize issues, resolve conflicts and communicate across all levels of the organization
Should be team-oriented and motivated to contribute significantly to scaling care management offerings for ambulatory patients at HSS
Must be able to learn new software tools and systems as required
Proactive, detail-oriented, pragmatic, and energized by a fast-paced, evolving environment
