Care Navigator - Bilingual, Korean
The CareBridge Care Navigator supports and collaborates with both patients and medical professionals to ensure effective, organized, and comprehensive care for patients with physical, developmental and intellectual disabilities. This is a frontline role which partners with an interdisciplinary care team to provide virtual disease education and care coordination to patients. This role conducts regular telephonic and televideo visits with patient, assists patients in scheduling visits with other interdisciplinary care team members, and facilitates care coordination with other medical providers.
Responsibilities:
Check in with patients on a regular basis to see if they have any immediate care needs and to assist with disease education and care coordination
As necessary, schedule patients with other interdisciplinary care team members
Arrange for prescribed laboratory tests, specialist referrals, and/or diagnostic procedures
Provide day to day support to assigned care team which includes RNs, NPs, Pharmacists, Social Workers, etc.
Communicate with patients about their condition through email, phone, and video conversations
Maintain excellent punctuality and attendance during work hours and adhere to schedule
Manage large amounts of inbound and outbound inquires and document all correspondences within systems timely and accurately, all while targeting for a first call resolution

Qualifications:
Fluent in Korean
Passion for helping patients with physical, developmental and intellectual disabilities
A warm, engaging personality with the ability to connect with individuals virtually via phone and/or video
Experience in a healthcare environment preferred
Experience delivering persuasive messaging and empathy
Expertise in phone etiquette and professional phone demeanor
Solution-oriented and a creative problem solver
Inspires a positive and collaborative working environment
Ability to work in a metric-driven and goal-oriented environment
Experience with conflict resolution, defusing, and de-escalating calls
Calm, unhurried approach that inspires confidence
Highly organized and detail-oriented
Self-starter with a propensity to follow-up and follow-through
Experience with Thrio and Elation systems preferred
