Primary responsibilities are:
Gather and review eligible beneficiary's self-reported health information
Update the eligible beneficiary's medical, social and family history
Review beneficiary's Health Risk Assessment to identify potential barriers, risk factors, opportunities for education and gaps in recommended preventive services
Counsel beneficiary on age-appropriate preventive services based on the beneficiary's health status and screening history
Furnish personalized health advice to the beneficiary and a referral as appropriate, to health education or preventive counseling, services or programs
Update the EMR to reflect the beneficiary's most current information

Job Requirements
Current RN nursing license in the state of Ohio; prefer licensure in Michigan and Ohio
CPR certification
Bachelors or Master degree in nursing from an accredited school
Critical thinking and clinical competence
Effective interpersonal and communication skills
Knowledge of insurance, federal regulations preferred
Flexible with hours to met the needs of the department
Computer literacy including inputting and retrieving information from the computer
Multitasking and organizational skills
Mobility and ability to move around department with prolonged periods of standing
