The Care Manager Specialist will use a person-centered model to provide care management services to Medicaid eligible members who exhibit two chronic health conditions or one single qualifying condition and special populations (Children and HH+ SMI). The Care Manager Specialist will coordinate with Managed Care Organizations and providers to develop and implement person centered plan of cares. The Care Manager will oversee and provide access to all of the services the member needs in an effort to stay healthy, out of the emergency room, and out of the hospital. The Care Manager will determine eligibility, perform assessments, complete UAS assessments (HARP and CANS-NY), create a Plan of Care, provide comprehensive transitional care, complete referrals to community and social supports, and link services through Health Information Technology (HIT).Minimum Education & ExperienceOne of the following:1.Bachelors of Science/Arts degree: Qualifying education includes degrees featuring a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field2.A NYS teacher's certificate for which a bachelor's degree is required3.A Credentialed Alcoholism and Substance Abuse Counselor (CASAC)4.Current registration as a Registered Nurse and 2 years of relevant experience5.A Master's Degree with 1 year relevant experiencePlus4-5 years of experience: Experience should be relevant to the skills needed to deliver the 6 Core Services and should include one of the following:*
* Providing direct services to people with Serious Mental Illness, developmental disabilities * Substance use disorders or linking individuals with Serious Mental Illness, developmental disabilities, or substance use disorders to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services). * Experience working with children and familiesAND* Experience working with Managed Care Organizations and/or insurance providers * Experience working with electronic health record systems* A master's degree in one of the qualifying education fields may be substituted for one year of Experience.* A Valid NYS Driver's License with access to timely and reliable transportation to transport self and occasionally others as needed
