Care Manager RN in CASE MANAGEMENT at St Mary Medical Center-Walla

Under the general guidance and direction by the Supervisor and Director of Care Management, provides case management in accordance with policy and procedures of PSMMC in collaboration with the patient and patient's family

Required Qualifications:

• Associate's Degree in Nursing

• Washington Registered Nurse License upon hire

• 2 years Medical/Surgical nursing practice or a balance of RN case management and/or community resource planning experience

Preferred Qualifications:

• Bachelor's Degree in Nursing
