Care Manager RN

Description Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients.

The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients.

The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care.

Preferred Qualifications: Bachelor’s Degree in Nursing. Required within 3 years of hire date CCM upon hire. 3 years Experience in community health nursing or case management.
