SUMMARY:

The Care Manager RN is responsible coordinate day to day activities within the infusion center including but not limited to scheduling, acting as a liaison between UCHealth oncology staff and EPH, helping to facilitate new patient starts in the outpatient infusion center via external referrals. Responsible for ensuring that all requirements for receiving infusion care are met. Will act as a lead in the unit and a representative on required clinical meetings and committees.

ESSENTIAL DUTIES:

  • Coordinates the transition of care between Oncology team at UCHealth - including but not limited to social work, pharmaceutical care, patient scheduling, authorization, and order validation/transfer.

  • Serves as the contact point, advocate, and educational resource for patients, as well as their care team, family, and caregivers.

  • Works collaboratively with physicians, interdisciplinary teams, individual clients, and families to promote positive patient outcomes. Facilitates communication and coordination between members of the health care delivery team, involving the client in the decision-making process in order to minimize fragmentation in services.

  • Performs a care continuum process that assesses, plans, implements, coordinates, monitors and evaluates the options and services required to support the individual's health needs utilizing skilled communication, education, and resources to promote quality, cost-effective outcomes. Collaborates with directors, physicians, and clinical staff related to patient access.

  • Demonstrates critical thinking skills when utilizing the nursing process, based on research, evidence-based outcomes and Standards of Practice to meet client's health care needs.

  • Provides on-unit coordination of care including but not limited to triaging patient, provider, or care team needs, communicating amongst care team for needs of patient, coordination of authorization, transition of care to EPH, making social work referrals, receiving critical communications from various areas of the treatment/care team.

  • Maintains and completes tasks associated with the new patient flowsheet, EPIC In Basket, EPIC Secure Chat, voicemail, scheduling, etc.

  • Coordinates external services needed such as line placement, palliative services, etc.

  • Facilitates new order procurement including from external providers. Answers questions regarding input of treatment plans.

  • Performs unit needs from a lead position - answers phone and responds to voicemail, responds to patient concerns/issues and elevates as needed, responds to unit communication within Epic (Secure Chat and In Basket), coordinates needs with other departments (lab, pharmacy, PASC, physicians clinic, etc).

  • Performs schedule scrubbing for the day and for the next week - identifying issues with orders, authorizations, medication procurement, etc.

  • Assign next day acuity to nurses for patient care coordination on the unit.

  • Leads daily unit huddle with staff members, authorization team, and pharmacy.

  • Assess patient charts regarding readiness for upcoming appointments including order signature, authorization, lab orders needed/completed, appointments scheduled appropriately, plan of care remains the same (via last progress note and recent triages) and send triage messages as necessary.

  • Helps with service recovery for patient issues.

  • Help staff with questions as they arise.

  • Attends the quarterly oncology meeting with UCHealth.

  • Participates in the Therapy Plan Committee through UCHealth.

  • Assists Director of Pharmacy with quality metrics and DNV tracers as assigned.

  • Approves Kronos in the absence of the Director of Pharmacy.

  • Assists as needed with creation of the schedule.

  • Other duties as assigned by the Director of Pharmacy.

POSITION REQUIREMENTS:

  • Minimum of two years in oncology nursing or leadership; inpatient and outpatient experience preferred.

  • Must possess a valid Colorado RN license or a valid multistate RN license through the Nurse Licensure Compact.

  • American Heart Association BLS required on hire.

  • AHA ACLS required within 6 months of hire.

  • ONS/ONCC Chemotherapy/Biotherapy certification. ONCC preferred. Required within 6 months of hire.

PAY RANGE:

$38.91 - $60.31 per hour
