Care Manager Registered Nurse (Rn)

CTTX Health is looking for a compassionate and dedicated Registered Nurse to join our team as a Care Manager. In this role, you will be responsible for providing comprehensive and personalized care management services to our patients. We are seeking an individual who is passionate about patient care and has a strong background in nursing.

As a Care Manager, you will work closely with patients, their families, and other healthcare professionals to develop and implement care plans that meet the unique needs of each patient. You will assess patients' health conditions, coordinate healthcare services, and ensure continuity of care. Additionally, you will educate patients and their families on self-management techniques and provide emotional support throughout the care process.

Requirements

• Valid RN license
• Bachelor's or Associate's degree in Nursing
• Minimum of 2 years of nursing experience
• Experience in care management or case management preferred
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively as part of a multidisciplinary team
• Excellent organizational and time management skills
• Passion for providing patient-centered care
• Knowledge of healthcare regulations and guidelines
• Basic computer skills
