SUMMARY
The Lead Care Manager II-Level I directs and manages the clinical activities and outpatient services of Care Manager Staff and Recovery Support Specialist staff while working with the Regional Program Director (RPD) to assure contract compliance of assigned programs. Ensure the implementation of the comprehensive care plan, which will include mental health goals, physical health goals, and other life domain goals for achievement of clinical outcomes consistent with the needs and preferences of the client. Assists in planning, organizing, and directing the admissions of clients. Will be responsible for the supervision of staff, program development, program administration, staff training, and providing services as needed. The Lead Care Manager interacts regularly with other Care Managers, as well as all other treatment providers, to ensure that the guiding principles of case management practice for individuals with mental, emotional, or substance abuse problems, including abuse of alcohol tobacco, and/or other drugs are being carried out. The Lead Care manager is responsible for ongoing monitoring of client charts to ensure that follow-up services are being provided. Other activities may include crisis diversion, case management, treatment planning, client advocacy, prevention and education. Maintains knowledge and familiarity of the goals and objectives of the agency.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Maintains knowledge of agency goals and objectives and works concurrently toward these goals and objectives with the RPD by attending key meetings, preparing reports, etc.
Demonstrates excellent communication skills with other program leaders and staff in order to provide quality care to clients. Maintains positive communication linkage with outside supportive sources
Supervises Care Manager staff and Recovery Support Specialist staff
Plans and develops programmatic goals and objectives, seeking ways in which to work more efficiently and effectively
Maintains partial caseload, provides quality direct services to clients and may be referred most severe and critical clients, provide consultation for staff with most severe/complex cases
Ensures quality service to clients and accurate and timely processing of documentation
Prepares, plans, facilitates, and implements direct care of clients through groups, and individual case management and supportive services
Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
Performs other duties as assigned
QUALIFICATIONS
Certified as a Behavioral Health Case Manager II (BHCM II)
Bilingual preferred
General skill in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships and strategic alliances with consumers, other staff, the community and other non-profit organizations
Must complete all in-service and external training requirements
Basic computer skills
Valid Oklahoma Driver License, with a clean Motor Vehicle Record, and current Auto Insurance
Two or more years' experience working with community services and health agencies as well as working with low income and minority consumers with understanding of cultural diversity preferred
