Care Manager Lcsw *Per Diem/Days*

Provides clinical social work services on the Crisis Stabilization Unit (CSU) in a manner consistent with the philosophy and objectives of LCM-SPH and the NASW. The Social Worker is responsible for psychosocial assessments. Participates in treatment team planning, processing commitments, discharge planning/case management service, crisis intervention, and liaison services with referral sources and other departments within the Providence Health & Services and community.

Required Qualifications:

• Master's Degree in Social Work.

• Upon hire: California Clinical Social Worker License

• Within 1 year of hire: CPI certification (nonviolent crisis intervention training)

• 2 years of experience in healthcare related setting with psychiatric mental health patient population or within an emergency department working with psychiatric patients.

• Computer experience as needed for the job.

Preferred Qualifications:

• Discharge planning experience.
