WHAT YOU WILL DO
This is a .6 D/E with Holidays and Weekends
Position located at Carmel Facility
Utilize information from patient interview, patient's chart, written and verbal reports to make appropriate assessment based on information received.
Discuss Post OP care options with patient including options for physical therapy, care destination, insurance criteria for care, and needed medical equipment and assistive devices.
Use assessment information received to develop appropriate post-operative discharge care plan for patient.
Provide communication of specific patient needs and care plan to appropriate health care professionals via EMR documentation, emails, phone calls, and text messages.
Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment.
Collaborate with other departments and with staff within the department to develop, implement, and maintain efficient clinical programs.
Continue personal education to improve practice.
Keep up to date on insurance and CMS regulation and guideline changes.
WHAT IT TAKES TO SUCCEED
Prior RN and/or Case Management experience.
Maintain composure, even in very difficult situations.
Customer service skills.
Healthcare Financing Administration (HCFA) Quality Management Guidelines.
Established Medical Practice Standards.
Knowledge of Managed Care requirements under the Medicare/Medicaid and other third party payor programs.
Develop effective relationships with patients, hospital departments, and other external parties.
Develop own ways of working effectively and efficiently.
Knowledge of Pharmaceutical terminology, generic and trade names, and ICD coding.
Knowledge of terminology used in diagnosis and classification of illnesses, injuries, and disabilities.
