Overview

Health Quest was formed through an affiliation of four hospitals: Northern Dutchess Hospital (Rhinebeck, NY), Putnam Hospital Center (Carmel, NY), Sharon Hospital (Sharon, CT) and Vassar Brothers Medical Center (Poughkeepsie, NY). Health Quest also includes affiliated healthcare providers Health Quest Medical Practice and The Heart Center. Together, these hospitals and healthcare providers have devoted themselves to the development of clinical specialties and medical programs and services.

We also count among our partners long-term care facilities, a free-standing radiation oncology center, urgent care centers, a multi-specialty medical practice and a home care service.

Qualifications

The care manager works as a member of the health care team to promote timely, comprehensive, coordinated, cost-effective, and efficient, quality care to all patients.

  • Assist health care team with patient risk stratification and risk identification.

  • Conduct comprehensive health, educational and psychosocial patient assessments.

  • Assist patients with attaining social services and transportation services.

  • Collaborate with providers to determine appropriate level of care management required for patients.

  • Coordinate and facilitate patient/family access to community resources and services.

Registered Nurse licensed in the state of New York.

Bachelors of Science in Nursing preferred.

Experience:

Three (3) years' experience in clinical nursing, case management, primary care, and/or home care.

Skills and Abilities:

Problem solving, critical thinking, time management, and organizational skills.

Excellent verbal and written communication skills.

Comfortable with data and using data when developing care plans.

Familiarity with eClinicalWorks EHR preferred, but not required.

Full-Time Day M-F, 7am-6pm; hours may vary

Join us and start doing yourlife's best work.SM Primary Responsibilities: * Utilizes the PAI system to complete complex elective, urgent and / or preregistration worklists of accounts * Contacts insurers to verify patient's insurance benefits, utilizes insurance websites and other means of electronic insurance verification (HDX) to obtain all information necessary for securing patient accounts. (Coordination of benefits, benefit coverage / limits, copay / deductible amounts, and authorization.) * Obtains and accurately records patient demographic and insurance information in the hospital's registration system and in PAI to facilitate appropriate billing for multiple units. * Secures authorizations for admissions and procedures when necessary. * Communicates professionally and courteously with patients and family members while obtaining or clarifying demographic information and / or insurance benefits and/ or financial responsibility. * Communicates the Hospitals collection policy and collects self pay and copay obligations. Utilizes to collect payments. * Educates uninsured / underinsured patients about the financial assistance program. * Insures high quality information is recorded in systems at all times Required Qualifications: * High School Diploma / GED (or higher) * 1 years of hospital front end registration and/or intake experience * Must have availability to work part time with rotating shift 3 days one week and 4 days the following week (including weekends) working 52.5 biweekly (26.25 / hr per week) PreferredQualifications: * Proficient with Microsoft Outlook (emails and calendar) * Knowledge of medical terminology, and a broad understanding of medical insurance laws and guidelines, insurance policy, coverage types, hospital billing procedures and payment policies, required.
