Village Place Health and Rehabilitation Center has an opportunity for a Care Manager covering the Greater Port Charlotte area. The Care Manager will be responsible for the business development and implementation of the market plan in the Greater Port Charlotte area. The Care Manager will conduct market assessments and develop a comprehensive sales and marketing plan designed to promote community, customer, payer and patient understanding of Village Place Health and Rehabilitation Center as the number one skilled nursing and long-term care healthcare provider in the area. We need a superstar Care Manager that can influence decision makers, develop strong relationships with the local healthcare community's physicians and hospitals, and will help develop community plans to drive brand recognition.

Requirements for the Care Manager position:

  • Licensed Nurse with strong ties to hospitals and physicians in the community.

  • Proven track record in health care sales preferred. Experience in medical device and/or pharmaceutical sales strongly preferred.

  • Minimum of a college degree in Marketing, Business or Allied Health Field.

  • Knowledge of regulatory standards and compliance requirements.

  • Ability to market aggressively and deal tactfully with customers and the community.

  • Demonstrates good communications, negotiation, and public relations skills.

  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

  • Persuasive selling skills

  • Value- Based selling Knowledge

  • An existing book of business and strong relationships with MD's and hospitals in the Broward Community

  • Pharmaceutical Sales experience is a plus

Summary Clinical Home Care Coordinator is primarily responsible for reviewing all homecare orders, certifications, and recertifications for accuracy, contacting patients to gauge level of satisfaction with designated homecare agency, communicate with PCP regarding orders, obtain signatures from PCP, document and submit billing as appropriate, and work with other care coordinators. S/he must exhibit strong interpersonal and communication skills as well as the ability to utilize both internal and external resources to accomplish the goals and objectives. Essential Duties and Responsibilities include the following. Other duties may be assigned. * Review all homecare certifications, recertifications, and orders received from homecare agencies and review for accuracy to meet patients needs * Communicate with patients, PCP and specialists * Obtain signatures from PCP on documentation * Complete all necessary documentation for billing requirements * Work with homecare agencies to coordinate care as needed * Meet with homecare agencies within the designated area to build relationships as needed Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
