Position: Care Manager

Reports to: Lead Supervisor

Position Type: Full Time, Exempt

Office Location: Fort Atkinson, WI (During initial training period, role then becomes relatively remote)

Location of Caseload Coverage: Rock County

SUMMARY:

As a member of an Interdisciplinary Team, you will provide on-going care management of adults with disabilities and frail elders through regular face-to-face and phone contact, following the standards set forth by the Managed Care Organization. You will develop and maintain care plans that assist Members to live and function as independently as possible. Collaborating with the Family Care RN on the Interdisciplinary Team to assure the health and safety of Members enrolled in the Family Care program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Complete a comprehensive psychosocial assessment, identify and mitigate potential health and safety concerns, and develop a member-centered plan of care collaboration with the Member and Registered Nurse focused on achieving and sustaining Member outcomes and fostering independence.

  • Encourage Members to take an active role in decision-making regarding their long-term care plan designed to assist them to live and function as independently as possible at home and in the larger community.

  • Through the development and evaluation of the member-centered plan, identify Member outcomes and the most effective and cost-effective interventions, including paid and unpaid services, needed to support those outcomes.

  • Provide assistance to Members and their families to access resources identified to support outcomes.

  • Coordinate and authorize services in the benefits package that are necessary and appropriate to support Member outcomes in the most cost-effective manner as well as oversight of supports provided through natural supports.

  • Oversee the effectiveness of the established member-centered plan and interventions through regular phone and in-person contact with the Member and their support system. Re-evaluate and adjust the plan during major life changes to assure to plan and interventions are tailored to the unique needs of the Member.

  • Respond to and report any event, incident, action, or in-action that jeopardizes the health and safety that involves a Member to the MCO and establishes a plan to prevent future reoccurrences.

  • In crisis situations, provide an appropriate assessment of psychiatric, behavioral or other concerns, and implement interventions in conjunction with the Registered Nurse, Supervisor, additional MCO staff, Member supports, the Members physician and/or other community support professional.

  • Maintain accurate documentation of care management activities in Member records promptly, thoroughly and professionally in accordance with the standards set forth by the MCO.

  • Participate as needed in MCFCs quality assurance and utilization review programs and follow quality standards of practice established by the MCO.

  • Attend team meetings, staffings, staff development programs, and in-service training and periodically collaborate with professionals who have additional specialized expertise when needed to assist in assessment, consultation, ongoing coordination efforts, and other areas as needed.

EDUCATION and/or EXPERIENCE:

  • Bachelors degree from an approved college or university required with a major in a social services area (e.g. social work, rehabilitation, psychology) or a Wisconsin social worker certification.

  • At least one (1) year of paid formal (non-family setting) experience working with specific populations (e.g. IDD, PD, Frail Elder) in social service care management or related social service experience with persons in the MCOs target population.

  • For a bachelors degree in any other area, the candidate must have a minimum of three (3) years of paid formal (non-family setting) experience with people in the MCOs target population.

  • Valid Drivers License with access to a reliable vehicle

  • Automobile insurance coverage (at time of application and throughout employment) of at least 100/300

Collaborating with the Care Manager on the interdisciplinary team, you will assure the health and safety of Members enrolled in the Family Care program. ESSENTIAL DUTIES AND RESPONSIBILITIES: * Complete a comprehensive nursing assessment, identify and mitigate potential health and safety concerns, and develop a Member-centered plan of care collaboration with the Member and Care Manager focused on achieving and sustaining Member outcomes, fostering independence, and the best possible health for the individual; * Through the development and evaluation of the Member centered plan, identify Member outcomes and the most effective and cost-effective interventions, including paid and unpaid services, needed to support those outcomes; * Provide assistance to Members and their families to access resources or medical care identified to support outcomes. Coordinate and authorize services in the benefits package that are necessary and appropriate to support Member outcomes in the most cost effective manner as well as oversight of supports provided through natural supports; * Evaluate Members' ability to safely manage, either by themselves, or through use of supports, their medication, medical care interventions and treatments, medical equipment, and access to health care services and educate as needed; * Take appropriate measures to prevent the spread of infection and communicable diseases and instruct the Member and/or caregiver in these measures; * Ensure that the Member's physician or appropriate medical professional is notified when the Member has an acute change in condition; * Re-evaluate the Member's health and wellness, based upon Family Care standards and nursing best practices, to manage ongoing health and safety issues; * Coordinate with primary and acute care systems for Members; * Monitor and follow-up with Member hospitalizations; * Perform routine health monitoring activities, based on the Member's need and MCO standards, for example, reviewing blood sugar monitoring results; * Establish and monitor wellness and health promotion activities based on the Member's assessment and identified personal and clinical outcomes; * Respond to and report any event, incident, action, or in-action that jeopardizes the health and safety that involves a Member to the MCO and establishes a plan to prevent future reoccurrences; * In crisis situations, provide an appropriate assessment of psychiatric, behavioral or other concerns, and implement interventions in conjunction with the Care Manager, Supervisor, additional MCO staff, Member supports, the Member's physician and/or other community support professionals; * Maintain accurate documentation of care management activities in Member records promptly, thoroughly and professionally in accordance with the standards set forth by the MCO; * Support a Member's continued eligibility for the Family Care benefit through maintenance of an accurate assessment on the Long Term Care Functional Screen; * Participation as needed in MC-CW's quality assurance and utilization review programs and follow quality standards of practice established by the MCO; * Attend team meetings, staffings, staff development programs, and in-service training and periodically collaborate with professionals who have additional specialized expertise when needed to assist in assessment, consultation, ongoing coordination efforts and other areas as needed. EDUCATION and/or EXPERIENCE: * Licensed Registered Nurse by the State of Wisconsin * Bachelor's Degree in Nursing preferred, but not required * Valid Driver's License, access to a reliable vehicle and automobile insurance coverage throughout their tenure. While this role can be performed relatively remote, business needs require the successful candidate to live within a reasonable distance to the county of coverage as home visits are part of the responsibilities.
