Description:
Responsible for assessment, member-centered care plan development, ongoing care coordination, quality assurance and monitoring of services to program members, who may be older adults and/or individuals who have intellectual and/or physical disabilities.
Achieve member outcomes and goals through adherence to standards of care and best practices
interact with members and their families with the highest level of respect, compassion and integrity
Identifies and/or participates in efforts to continuously improve the quality of member care and services
Fosters the member-centered culture
Complete discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults with families and other supports for additional information as needed
Develops and maintains care plans, including members' long term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports
educates members and families regarding aging, disability or health issues, and provides information to members to assist in establishing and maintain community links. Assists members with filing complaints, grievances or appeals and obtaining advocacy services as needed.
Maintains member contact standards and documentation standards, and stays informed of current trends and best practices
Manages an assigned caseload of program members in collaboration with an RN Care Manager
Completes comprehensive member assessments, member centered care plans, & functional screens
Provides ongoing care coordination and quality assurance monitoring of services
Maintains required documentation in adherence to established standards.

Additional Skills & Qualifications:
Education: Bachelor's Degree in a Human Services Area or closely related field required.
Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required.
Certification: Eligibility for long-term care functional screener certification is required
