Position Summary:

Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

Reports to: Director of Care Management

FLSA: Exempt

Minimum Qualifications:

Minimum Education

Bachelor's degree in Social Work required. Master's degree in Social Work preferred

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervisor.
