Responsible for managing overall operational leadership and department functions of RN Care Coordinators Inpatient. Responsible for: developing strategies and operational plans for length of stay management, patient throughput, and appropriate utilization of resources, ; fostering positive staff, physician and interdisciplinary collaboration; and supporting unit policies/procedures and providing support to special programs and committees.
Provides direction, leadership, management and oversight of the department.
Strategic and operational planning, implementation and evaluation to ensure effective patient care management.
Participates and leads process improvement projects to address system-wide delays and improve quality clinical coordination and patient management
Supervise staff and give technical direction, monitor and prioritize workflow.
Manage the selection, training and development of staff which includes performance assessments.
Works with physicians, medical social workers, nursing unit managers and educators to provide effective patient care coordination.
Ensures department compliance with all regulatory standards and leads staff to a culture of safety, continuous improvement and quality patient care.
Particiaptes in the budgeting and budget monitoring processes.
Performs other duties as assigned.
Minimum of two years management experience in healthcare organization preferred.
Solid leadership background preferred.
Current Registered Nurse license in state of practice required.
Certification in Case Management required within two years of hire.
Strong leadership, needs assessment, program development, program development, project management and organizational alignment skills.
Strong problem solving and decision making skills.
Strong communication skills, both verbal and written.
Strong training and facilitation skills.
Proficient computer skills including MS Office applications.

RIVERBEND PHYSICAL/COGNITIVE REQUIREMENTS:
Work requires fairly light physical exertion up to 33% of the time.
Ability to lift objects weighing 50 lbs. or less.
Work requires frequent exposure to minor cold, heat, poor ventilation or sharp instruments. Reasonably anticipated exposure to blood and body fluids once per month or more.
Job duties frequently require intense concentration and attention to detail (34-65% of work time).
