Job Description:


Support comprehensive coordination of medical services including Care Team intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services.

Fundamental Components:

Care Management Team Intake

  • Responsible for initial review and triage of Care Team tasks

  • Identifies principle reason for admission, facility, and member product to correctly apply intervention assessment tools

  • Screens patients using targeted intervention business rules and processes to identify needed medical services, make appropriate referrals to medical services staff and coordinate the required services in accordance with the benefit plan

  • Monitors non-targeted cases for entry of appropriate discharge date and disposition

Support the Development and Implementation of Care Plans

  • Coordinates and arranges for health care service delivery under the direction of nurse or medical director in the most appropriate setting at the most appropriate expense by identifying opportunities for the patient to utilize participating providers and services

  • Promotes communication, both internally and externally to enhance effectiveness of medical management services (e.g.,health care providers, and health care team members respectively)

Adheres to Compliance with PM Policies and Regulatory Standards

  • Maintains accurate and complete documentation of required information that meets risk management, regulatory, and accreditation requirements

  • Protects the confidentiality of member information and adheres to company policies regarding confidentiality

  • May assist in the research and resolution of claims payment issues

  • Supports the administration of the hospital care, case management and quality management processes in compliance with various laws and regulations, URAQ and/or NCQA standards, Case Management Society of America (CMSA) standards where applicable, while adhering to company policy and procedures


  • 2-4 years experience as a medical assistant, office assistant, health care support


The highest level of education desired for candidates in this position is a High School diploma, G.E.D. or equivalent experience.

Strong customer service skills to coordinate service delivery including attention to customers, sensitivity to issues, proactive identification and resolution of issues to promote positive outcomes for members.

Computer literacy in order to navigate through internal/external computer systems, including Excel and Microsoft Word.

Ability to effectively participate in a multi-disciplinary team including internal and external participants.

Familiarity with basic medical terminology and concepts used in care management.

Effective communication, telephonic and organization skills.

Department of Social Work (MSH) - Req Number: 2407678 Job Title: Social Worker – Care Management (BILINGUAL) Roles & Responsibilities: Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System's implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting. The Department of Social Work Services takes a leadership role in MSHP Care Management, which is providing in person and telephonic short-term and long-term care management services for a large diverse group of individuals who have a variety of insurances that are part of value based contracts. The goal of MSHP Care Management is to provide standardized care management services to meet patients' medical and psychosocial needs, increase quality and reduce unnecessary health care costs. The PeakHealth Clinic provides intensive medical and care coordination services to patients with multiple chronic conditions who are high utilizers of the Emergency Department and/or who have had multiple hospitalizations. It also provides short-term intensive medical management and care coordination services to patients with newly diagnosed medical conditions as well as to those who have recently been discharged from hospital or a nursing home. The Social Worker (PeakHealth Clinic) is responsible for providing assessment and interventions services to at risk patients, developing effective relationships with them and their family caregivers, and working collaboratively with the care team to improve health outcomes. These positions will be based in the Center for Advanced Medicine on 102nd Street (campus of the Mount Sinai Hospital) and may include home visits. Requirements: * New York State LMSW or LCSW. * Bilingual (English/Spanish). * Exceptional skills in engaging patients/families and in assessment and coordination of resources within families and hospital and community networks for effective management of patient care. * Exceptional organizational, collaborative and psychosocial assessment and intervention skills. * Interest in being part of an innovative initiative to improve the quality of care by addressing barriers to behavioral health, primary and specialist care, preventable Emergency Department visits and hospital admissions/readmissions. * Hospital experience and experience with patients with chronic behavioral health and medical conditions preferred. * Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications.
