Position Summary:

We have an exciting opportunity to join our team as a Care Management Assistant - Social Services.

In this role, the successful candidate Responsible for providing administrative support services to all staff in the Care Management Department. Processes correspondence related to all care management functions (i.e., communications from third-party payors, discharge appeals, etc.). Responsible for maintaining the integrity of the departmental care management databases (departmental server, Canopy/ECIN Care Management System). Prepares care management reports to aggregate the data to use as information for tracking, trending and problem identification.

Job Responsibilities:

  • Utilizes hospital information systems (ICIS, EDR, and EPASS) as they relate to Care Management.

  • Coordinates the arrangements for referrals for transportation, durable medical equipment, residential facility placements and other facilities as needed for all eligible patients.

  • Assists in obtaining authorizations for transition to the next level of care (i.e., home, acute/subacute rehab facility, transportation, DME, etc.). Contact health insurance provider for information needed for discharge.

  • Inputs essential data from third party payors into Canopy/ECIN Care Management or other software systems and/or the Patient Financial Services database.

  • Screens telephone and fax requests. Responds to inquiries and makes appropriate referrals.

  • Serves as central intake worker for variety of departmental functions (i.e., utilization management, discharge planning, etc.).

  • Performs other duties as assigned.

  • Orders/maintains an inventory of supplies for the department section. Makes arrangements to appropriate repair vendor when office equipment not functioning.

  • Collects, updates, and summarizes data and statistics as requested. Composes, prepares, edits general correspondence, and routes incoming mail to the appropriate section. Sets up and maintains proper documentation and files of all departmental and program activities.

  • Greets, directs, and responds to the needs and requests of the staff, patients, and general public.

Minimum Qualifications:

To qualify you must have a Graduate of a business/secretarial school or the equivalent combination of education and work experience. Minimum of three years of secretarial/administrative experience in a related setting.

Effective oral, written communication and interpersonal skills including ability to work collaboratively and cooperatively within a team and internal and external customers. Strong organizational skills and ability to set priorities. Must be able to manage a variable workload.

Must be flexible in a changing environment. Excellent written and verbal communication skills required. Proficient in the use of Microsoft Office.

Preferred Qualifications:

Associates or Bachelors degree preferred.

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $58,381.06 - $58,381.06 Annually.

Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
