Mission for Position

The Care Management Advanced Practice Provider will collaborate with interdisciplinary teams to design, implement and optimize care pathways that prioritize quality, efficiency and patient satisfaction. A compassionate and driven provider with a strong commitment to improving healthcare delivery and patient experiences, the Care Management APP plays a critical role in our efforts to improve patient outcomes while reducing costs through value-based care initiatives.

Position Summary

Strategic Priorities

  • Advocate for and lead change in value-based, care coordination initiatives across the system.

  • Coordinate the care of a variety of patients with a focus on preventative care and disease management within a value-based framework.

  • Collaborate with interdisciplinary teams, including care managers, nurses, pharmacists, and social workers, to develop and implement evidence-based care pathways that align with value-based care principles.

  • Utilize health information technology and electronic health records to track patient outcomes, identify care gaps, and monitor adherence to clinical guidelines.

  • Participate in system efforts to manage high risk/high cost patients through collaboration with team members.

  • Participate in quality improvement initiatives, performance measurement activities, and population health management efforts to optimize care delivery and achieve value-based care goals.

  • Stay abreast of emerging trends, best practices, and innovations in value-based care and contribute to the continuous improvement of care delivery models.

Day-to-Day Priorities

  • Provide comprehensive medical care to patients within a value-based care framework, focusing on preventative care, chronic disease management and transitional care.

  • Conducts assessments, which includes comprehensive annual wellness exams on non-paneled patients.

  • Engage with patients to promote shared decision-making, educate them about their conditions and treatment options, and empower them to actively participate in their care plans.

  • Additional day-to-day patient care duties include:

  • Hospital discharge follow-ups (average 3 patients per day)

  • Emergency department follow-up appointments as needed.

  • Facilitate closing gaps in care by educating patients about preventive monitoring and working with provider practices to schedule diagnostic testing.

  • Prescribe medications, order tests, and collaborate with patient's primary care providers to enhance care management and quality of life.

  • Support top of scope clinical guidance for entire Care Management team, including HCH, chronic care management, palliative care, and transitional care management teams.

  • Participate in the integrated care team meetings including Hospital Quad.

  • Maintain compliance with regulatory requirements, payer contracts, and quality reporting standards related to value-based care programs.

  • Support quality improvement and cost reduction strategies (e.g., hospital readmissions, unnecessary emergency room visits, duplicative services, unnecessary care) for attributed populations.

Qualifications

Qualifications

  • Board certified and hold a valid state registration or license.

  • 3 years of clinical experience in primary care or a related specialty

  • Strong understanding of value-based care principles, including risk stratification, care coordination and population health management.

  • Current and unrestricted DEA certificate

  • Must maintain certification of BLS and have current course credentials.

  • Is proficient in health information technology and electronic health records.

  • Has excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate effectively with interdisciplinary teams.

  • Maintains commitment to continuous learning, quality improvement and patient-centered care.

  • Has experience with value-based care contracts, payer relationships and quality reporting preferred.

  • Can work without direct supervision and practice autonomously
