Join the UVU CARE Hub and play a key role in providing essential support and resources to our vibrant student community! The program manager, case management, has a key leadership role within the CARE Hub, reporting to the director. This position is responsible for the overall design, successful management, and implementation of student case management services. This individual will play a pivotal role in overseeing the allocation of resources, collaborating with student clients, and ensuring compliance with program guidelines to support individuals seeking basic needs support. This individual will oversee the emergency scholarship fund, develop standard operating procedures, and manage community partnerships.

  • Manage a comprehensive caseload of student clients, assessing their needs and developing individualized plans to address housing, healthcare, and other support services. Conduct intake interviews to determine clients' requirements and eligibity for services. Provide personalized case management, including follow-up and ongoing support, to ensure clients achieve their goals. Assess clients' needs, situations, strengths, and support networks to determine their goals.

  • Design and implement case management policies and procedures that align with the CARE Hub's mission and goals. Develop schedules and routines for clients to ensure they receive consistent and comprehensive services. Coordinate with various service providers on- and off-campus to arrange necessary resources and support for clients.

  • Establish and maintain relationships with community organizations, campus entities, housing authorities, and other relevant stakeholders. Act as a liaision between clients and external service providers, ensuring effective communication and coordination. Conduct outreach and awareness campaigns to promote case management services on-campus. Advocate on the client's behalf to community organizations and campus entities.

  • Maintain accurate and up-to-date records of client interactions, service plans, and outcomes. Prepare and submit detailed reports on program activities, client progress, and performance metrics. Ensure compliance with all relevant regulations and guidelines related to case management services. Evaluate clients' progress periodically and make adjustments as needed to improve outcomes.

  • Perform other job-related duties as assigned.

Bachelors in social work, communication, community health, or related field OR a combination of education and experience totaling four years. Demonstrated ability to establish and maintain collaborative working relationships, demonstrated experience working effectively on a team, demonstrated commitment to public service, and working with a variety of individuals.

Preferred Qualifications:   

A MSW or related degree.Knowledge

  • Knowledge of case management principles, practices and techniques.

  • Understanding of relevant federal, state, and local regulations related to housing and support services

Skills

  • Strong organizational, verbal, and written communication skills.

  • Proficiency in database management and assessment tools.

  • Ability to plan, coordinate, implement, and evaluate case management programs.

Abilities

  • Ability to effectively manage a moderate caseload and provide individualized support to clients.

  • Ability to establish and maintain strong relationships with community partners and stakeholders.

  • Ability to educate clients and community members about available services and resources.

  • Strong problem-solving and decision-making skills.
