Care Coordinator Support

Role Summary:

This individual is considered one of the most important parts of the healthcare care team. They will work on the phone with patients to ensure that patients are following their physicians care plan, as well as collaborate with our Care Coordinators to arrange for the highest level of quality patient care.

Key Responsibilities:

  • Regular phone contact with patients in the Care Coordination program

  • Liaison with Care Coordination nurses regarding patients care. Escalates clinical calls, as necessary

  • Assist in identifying patients needing additional services and help to coordinate services

  • Facilitate referrals to outside providers

  • Schedule patients with Think providers

  • Send appropriate medical records to outside providers

  • Follow up on outcome of referrals

  • Accurate documentation in the electronic health record

  • Attendance and punctuality are requirements of the role

  • Behave in a way which is consistent with the mission and values of Think

  • Supports and behaves in a way that aligns with our code of conduct

  • Other duties as assigned

Skill, Knowledge, and Experience:

  • 2+ years of experience in a clinical setting, Family Practice or Internal Medicine ideal

  • Certified Medical Assistant

  • Certified Medicine Aide

  • CPR certified

  • 2+ years of experience with electronic health record, Allscripts ideal

  • Proficient computer skills

  • Strong service orientation/patient care

  • Strong communication
