We are looking for an experienced Medical Assistant to provide care coordination for the GI department. Must be a WA State Certified Medical Assistant MA-C.

Provides complex and diversified support to the Care Team (provider, medical assistant, nurse, etc.) by coordinating care for patients on the daily schedule; this support will require advanced administrative skills and knowledge. Will coordinate services for all patients who are part of the assigned panel. Focus on health maintenance and those with serious, complex, or chronic health problems, or those with psychosocial issues. Work from a team approach, and in continuous partnership with patients and providers to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being. Work is generally performed independently requiring judgment and problem solving skills with limited supervision.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Serve as an effective communication link between patient and clinic staff by gathering information from patients.

  • Independently and proactively identify patients in the practice by running panel reports for specific diagnosis and preventive care needs.

  • Communicates with patient directly to discuss preventative care needs and refers patients with medical concerns to appropriate clinical staff.

  • Serve as a medical home quality improvement team member, assist with improvement of work flows through PDSA Cycles and measurement of quality indicators.

  • Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.

  • Evaluate and analyze patient records based on primary Care patient standards for patients who need: preventive services, diagnostics, and follow up.

  • Schedule patients for preventive care needs.

  • Participate in team huddles and evaluation of team data for proactive panel management.

  • Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients.

  • Additional clerical duties as assigned.

EDUCATION AND SKILLS

  • WA State Medical Assistant-Certified.

  • Current Healthcare provider CPR certification, American Heart Association BLS.

  • At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.

  • Effective communication and active listening skills.

  • Demonstrated effective interpersonal and customer service skills.

  • Knowledge of medical terminology.

  • Philosophy and values consistent with a patient-centered care model.

  • Ability to work proactively and effectively in a team as well as independently.

  • Good organizational skills.

  • Demonstrated problem solving skills in a complex environment.

  • Proficiency working in an electronic medical record system and Microsoft Outlook, Work, and Excel.

ESSENTIAL DUTIES AND RESPONSIBILITIES * The Resident Care Manager plans and directs ongoing care in collaboration with the RN Care Manager shift supervisor, recognizing individual needs of residents and staff. * Monitors work assignments, provides feedback, evaluates performance, redirects staff, and informs Director of Nursing Services of pertinent problems timely. * Communicates and coaches staff regarding policies, completes quality care audits, and works with professional staff in the daily delivery of resident care. * Assist with new admissions, transfers, and discharges ensuring related documentation is current, signed, and the recordkeeping meets the policies and procedures. * Reviews and completes resident records and related documentation in a timely manner, including the completion of the MDS and observations/forms as scheduled for each resident * Plans and conducts resident care conferences and develops interdisciplinary plan of care for each resident. * Regularly observes, audits and reviews each resident's condition, status, and care requirements and takes appropriate action to assure care needs are met. * Develops nursing care plans for each resident that meets the needs of the resident and the physician recommendations and reports changes in condition of residents that requires nursing intervention notifying the physician, director of nursing services, and family. * Transcribes and carries out or delegates physician orders in accordance with the nurse practice act. * Documents all pertinent information in the interdisciplinary notes and on the 24-hour report timely and accurately. * Performs other duties as assigned.
