We are looking for an experienced Medical Assistant to provide care coordination for the GI department. Must be a WA State Certified Medical Assistant MA-C.
Provides complex and diversified support to the Care Team (provider, medical assistant, nurse, etc.) by coordinating care for patients on the daily schedule; this support will require advanced administrative skills and knowledge. Will coordinate services for all patients who are part of the assigned panel. Focus on health maintenance and those with serious, complex, or chronic health problems, or those with psychosocial issues. Work from a team approach, and in continuous partnership with patients and providers to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being. Work is generally performed independently requiring judgment and problem solving skills with limited supervision.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Serve as an effective communication link between patient and clinic staff by gathering information from patients.
Independently and proactively identify patients in the practice by running panel reports for specific diagnosis and preventive care needs.
Communicates with patient directly to discuss preventative care needs and refers patients with medical concerns to appropriate clinical staff.
Serve as a medical home quality improvement team member, assist with improvement of work flows through PDSA Cycles and measurement of quality indicators.
Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
Evaluate and analyze patient records based on primary Care patient standards for patients who need: preventive services, diagnostics, and follow up.
Schedule patients for preventive care needs.
Participate in team huddles and evaluation of team data for proactive panel management.
Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients.
Additional clerical duties as assigned.

EDUCATION AND SKILLS
WA State Medical Assistant-Certified.
Current Healthcare provider CPR certification, American Heart Association BLS.
At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.
Effective communication and active listening skills.
Demonstrated effective interpersonal and customer service skills.
Knowledge of medical terminology.
Philosophy and values consistent with a patient-centered care model.
Ability to work proactively and effectively in a team as well as independently.
Good organizational skills.
Demonstrated problem solving skills in a complex environment.
Proficiency working in an electronic medical record system and Microsoft Outlook, Work, and Excel.
