Job Description

Phoenix Home Care's Springfield, MO is in search of a Care Coordinator to join our team!

This position is responsible for scheduling and coordinating the day-to-day care delivered by field staff members who are working in the homes of clients. This position offers the right candidate a challenging position with opportunity for growth.

Location: Springfield, MO

Job Details:

  • Scheduling hours to be delivered in the homes of our clients.

  • Manage ongoing relationships with clients, client families and field staff.

  • Interviewing potential field staff employees

  • Troubleshooting problems and providing resolution


  • Ability to appropriately staff caregivers with clients

  • Ability to handle a multi-line phone system

  • Highly Organized & Detail Oriented

  • Available to cover shifts and assist with admissions when needed

  • Prior home care caregiving experience is preffered.

  • Must have prior office experience.

Opportunity Under the direct supervision of the Administrator, this position will promote and market our community. This position will: * Utilize both the clinical and business aspects of the position * Be accomplished by formalizing partnerships with Acute Care providers and local referral sources * Will have a flexible schedule to participate in scheduled and scheduling of meetings to best accommodate those we are trying to serve * Will maintain knowledge of our community and Americare policies and procedures The Post Acute Care Coordinator must be capable of communicating, meeting with and providing information to our Regional Operations Director, Regional Nurse Consultant, Corporate Managed Care Coordinator and the Facility Administrator. This individual must have knowledge of marketing, business skills, a professional appearance and demeanor. There is substantial travel associated with this position as well. Qualifications and or Education: * Excellent supervisory and leadership skills required * Exceptional communication and public relation skills * Possesses accurate and professional documentation ability * Portrays a professional attitude and has good social skills in the community * Displays a professional behavior and appearance at all times * Detail-oriented with realization that negative behavior could impact our facility or company * Exceptional nursing assessment skills Licenses/Certifications: Must possess current RN or LPN license Responsibilities * Responsible for maintaining existing and growing new partnerships with Acute care hospitals * Responsible for maintaining existing and growing new partnerships with local referral sources: hospitals, Physicians, Clinicians and ALF's. * Grow our Long Term and Rehab census * Responsible for scheduling, coordinating and presenting at C-Suite meetings * Responsible for calling on and building our relationships * Work with our Americare ALF's to build on our continuity of care. * Responsible for making contact with potential residents while they are in the hospital * Responsible for initiating advanced care planning with new admissions * Tracks all of our referrals and reports them to the Administrator and Regional Operations Director on a monthly basis * Will participate in local marketing of the medical and business community and participate in the local wagon wheel activities of the community * Follows up with our in-house rehab residents on a weekly basis and follows them once rehabbed to home on a weekly basis for one month * Attends weekly Medicare meetings * Attends the monthly LTC meetings * Attends the Local Chamber sponsored events and meetings * Will attend 1-2 daily stand up meetings * Monitors our readmission rates, reports and follows up on them with the administrator, DON and MDS coordinator Safety * Mandatory reporting of any work-related injury within 24 hours to the administrator * Participation in our drug testing procedures is a mandatory part of employment Americare Senior Living is a drug-free workplace Americare Springfield MO
