The RN Care Coordinator is a Registered Nurse who facilitates patient continuity of care with the healthcare team. Under the clinical direction and oversight of the Primary Care Physician, the Care Coordinator coordinates care for high risk/complex patients by collaborating with the patient, family, physician, nurses, and other members of the healthcare team to identify needs and expedite appropriate, cost effective care.

S/he assists in the development of the patient's care plan reviews LOS target outcomes and discharge plans with area facilities. . The Care Coordinator will serve as the patient's primary conduit to the primary care provider as they help coordinate necessary services both within and outside the practice, coordinating care from a variety of sources physicians, specialists, home care providers, rehab centers, pharmacists, etc. They will engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt life-long healthy behaviors.

Qualifications:

Education: Graduate of an accredited School of Nursing, Bachelors degree required, MSN preferred

Licensures & Certifications:

Valid Pennsylvania RN licensure required.

Bachelor's degree required

Case Manager (CCM) certification preferred

Experience:
5 Years Experience in clinical, utilization management, home care, discharge planning, and/or case management required
3 years experience in a managed care environment preferred

Ability to interact with physicians and other health care professionals in a professional manner required

Computer proficiency required.

Excellent verbal and written communication and interpersonal skills required

Ability to prioritize daily tasks and caseload activities to meet patient needs and turnaround. Ability to travel in the local or regional area depending on facility location. Ability to establish a home office work space

Strong problem solving, conflict resolution, and negotiating skills

Knowledge of managed are models financial reimbursement systems, clinical case management processes and utilization management issues

Knowledge of NCQA (PCMH) guidelines for care management

VIEW JOBS 1/9/2019 12:00:00 AM 2019-04-09T00:00 <p><strong>Embedded Care Coordinator (ECC) Role</strong></p><p>The ECC plays a pivotal role in the P4Q program by providing onsite practice support to assigned participating Primary Care Providers (PCPs) groups and health systems. <strong><em>The ECC role can be summarized in three (3) key areas of responsibility:</em></strong></p><ol><li>Partnership with PCPs / practice staff to improve member outcomes</li><li>Member care coordination and gap closure</li><li>Medical Record Review/Retrieval and data abstraction</li></ol><ol><li><strong>Partnership with PCPs / practice staff to improve member outcomes</strong><ul><li>Meet regularly with assigned physicians and their office staff to review progress reports for gap closure, and provide update on care coordination efforts. ECCs scheduled onsite visits are geared towards:<ul><li>Reviewing reports/ member details with PCPs/ office staff</li><li>Discussing barriers/ challenges in meeting program goals, identifying opportunities such as referral to other programs</li><li>Onsite chart reviews for data abstraction, in the absence of remote access</li><li>Participating in CHS Days (may assist in completing certain screenings, such as BP, BMI, Heel scan, etc.)</li></ul></li><li>Provide assigned physicians with detailed overview of P4Q program changes, as well as any market strategies / updates, as applicable</li><li>Travels to (and work out of) scheduled Primary care provider offices daily (usually just one per day, or more depending on space availability, proximity etc); Conduct targeted onsite /electronic medical record (EMR) chart reviews, and data abstraction as appropriate.May occasionally work in main office (O&rsquo;Donnell Street) if secured space unavailable in provider office to make calls etc.</li><li><strong>Practice Transformation: </strong>In partnership with Network operations team, ECCs assists physician practices in identifying areas of opportunity to improve quality care and outcomes, by sharing best practices, processes and workflows known to improve practice efficiencies and outcomes </li><li>Navigate multiple computer software systems including internal claims database, to assess customers care coordination needs</li><li>Meet weekly/monthly departmental productivity goals for number of outreach calls and gap closures</li><li>Assist in meeting departmental Special Needs Plan (SNP) model of care and other regulatory requirements as applicable</li><li>Attend other required trainings and meetings at main office, or offsite as required<p>&nbsp;</p></li></ul></li><li><strong>Members Care Coordination and Gap Closure</strong><ul><li>Performs telephonic outreach to assigned customers based on open P4Q/Star/HEDIS (Health Effectiveness Data and Information Set) gaps to coordinate annual preventive health screenings, transition of care/hospital discharge follow up and pcp visits, and address barriers to care</li><li>Provides member education (telephonic/ face to face in provider offices) on chronic disease management, such as Diabetes management, Hypertension (Blood pressure control), etc.</li><li>Meet face to face with members, in provider practices where possible, to educate, assist in gaps closures- such as during designated event days at targeted provider practices</li><li>Documents all customer outreaches in designated company systems</li><li>Collaborate with CHS internal Complete Health Team (CHT) to identify and coordinate care for members with avoidable high utilization, or at high risk.</li></ul></li><li><strong>Medical Record Review/Retrieval and Data Abstraction</strong></li></ol><ul><li>Reviews medical records (MR) for missing data or data not captured through claims, and enters data in designated internal system. Must pass quality audits for data entry with a score of 95% or higher.</li><li>Collects, scans and submits MR for annual HEDIS MRR retrieval process (every Feb-May)</li></ul><p>&nbsp;</p><p><strong>Requirements: </strong></p><ul><li>Current licensure as a Registered Nurse (RN), active and in good standing in applicable state <ul><li>Maryland: Must have Maryland RN licensure.
